超声刀痔切除术治疗重度脱垂性混合痔的临床研究

Clinical research of the harmonic scalpel hemorrhoidectomy in treatment of severe prolapse mixed hemorrhoids

目的探讨超声刀痔切除术在临床中应用的可行性和安全性。方法回顾性分析我院2011年3月—2012年3月收治的78例重度脱垂性混合痔住院手术患者,38例行超声刀痔切除术,40例行外剥内扎法痔切除术,比较两种手术方法的疗效。结果两组患者手术时间、术中出血量比较,差异有统计学意义(P<0.05、P<0.01);两组患者在术后尿潴留、术后疼痛比较,差异无统计学意义(P>0.05);肛门坠肿、肛缘水肿比较,差异有统计学意义(P<0.05)。结论与外剥内扎法痔切除术比较,超声刀痔切除术具有手术时间短、出血少、术后并发症少等优点。

Objective To explore the feasibility and the safety of the clinical use of the harmonic scalpel hemorrhoidectomy.Methods Seventy eight cases of severe prolapse mixed hemorrhoids in our hospital from March 2011 to March 2012 were analyzed retrospectively,38 of them underwent harmonic scalpel hemorrhoidectomy and 40 of them were taken Milligan-Morgan operation,then compare the efficacy of the two operation.Results The difference of operating time and amount of bleeding between two groups showed statistical significance（P0.05,P0.01）;there was no significant different in postoperation uroschesis and postoperation pain（P0.05）,the difference of postoperation anus falling and bulge and anus margin edema showed statistical significance（P0.05）.Conclusion Compared with the Milligan-Morgan,harmonic scalpel hemorrhoidectomy exceed in operating time,amount of bleeding and the postoperation complication.
