通脑活络针刺法对大鼠急性脑梗死细胞凋亡及梗死容积的影响

Impacts of Tongnao Huoluo Acupuncture therapy on cells apoptosis and infarction volome in rats with acute cerebral infarction

目的评价通脑活络针刺疗法对大鼠急性脑梗死的疗效。方法采用改进的微栓子法制造SD大鼠大脑中动脉血栓栓塞模型,将造模成功后的大鼠共264例随机分为≤1.5h组(72例)、1.5～2h组(72例)、2～3h组(72例)、缺血对照组(24例)、假手术组(24例)共5组。前3组分别予以通脑活络针刺疗法(针刺组)、尿激酶溶栓法(溶栓组)、单纯体针(体针组)治疗。治疗后24h、72h用TUNEL法测凋亡细胞计数、TTC染色测梗死容积百分比,并进行HE染色观察组织病理学变化。结果在2h时间段内,在24h、72h时点针刺组在降低梗死容积百分比、降低凋亡细胞计数方面,与溶栓组比较差异无统计学意义(P>0.05),且两者对上述指标的改善显著优于缺血对照组(P<0.01)及体针组(P<0.05);在2～3h时间段以内,针刺组对上述指标的改善显著优于缺血对照组(P<0.01),而溶栓组和缺血对照组比较差异无统计学意义(P>0.05),体针组和缺血对照组比较差异亦无统计学意义(P>0.05);≤1.5h针刺组对上述指标的改善显著优于1.5～2h针刺组(P<0.05),1.5～2h针刺组对上述指标的改善显著优于2～3h针刺组(P<0.05)。结论通脑活络针刺疗法在降低大鼠急性脑梗死梗死容积百分比及凋亡细胞计数方面有较好疗效,且越早治疗效果越好。

Objective To evaluate the effect of Tongnao Huoluo Acupuncture（THA） therapy on acute cerebral infarction（ACI）.Methods Adopting the improved micro embolus method to manufacture SD rats middle cerebral artery embolism mode.264 ACI SD rats were randomly divided into 5 groups：≤1.5h group（n=72）;1.5～2h group（n=72）;2～3h group（n=72）,ischemia group（n=24）;and sham operation group（n=24）.The first 3 groups were respectively treated with THA,thrombolysis with urokinase（TU） and body acupuncture.Therapeutic effect was evaluated by HE staining,TUNEL staining and TTC staining at 24h,and 72h time point.Results In 2h period,compared with TU,numbers of TUNEL-positive staining cells and the percentage of infarction volume in rats brain of THA were insignificantly different at 24h,and 72h time point（P〈0.05）,In this period,the above index of THA and TU were remarkably different from ischemia group and body acupuncture group（P〈0.05）.In 2～3h period the therapeutic effect of THA showed remarkable difference from TU,Ischemia group and body acupuncture group（P〈0.01）.Therapeutic effect of THA showed great difference in these 3 periods（P〈0.05）.Conclusion THA therapy shows favorable effects in decreasing the percentage of apoptosis cells and infarction volome in rats with acute cerebral infarctioninfarction.Therapeutic effect of THA is closely related with time window.
