术前静脉化学治疗对口腔鳞状细胞癌组织中COX-2蛋白表达及淋巴管密度的影响

摘要 [目的]探讨术前平阳霉素静脉化学治疗对高分化口腔鳞状细胞癌组织中COX-2蛋白的表达及淋巴管密度(LVD)的影响.[方法]选择45例口腔鳞状细胞癌患者术后完整包埋的石蜡标本,其中30例为单纯手术患者,15例为术前平阳霉素静脉化学治疗联合手术患者,用免疫组织化学PV-6000通用型二步法检测口腔鳞状细胞癌组织中COX-2蛋白表达与D2-40标记的LVD.[结果]静脉化学治疗组口腔鳞状细胞癌组织中COX-2蛋白阳性表达率明显低于未化学治疗组(P<0.05),静脉化学治疗组癌周平均LVD明显低于未化学治疗组(P<0.05),两组癌组织内平均LVD间差异无统计学意义(P>0.05),术前未化学治疗组COX-2蛋白表达阳性组癌周LVD与阴性组相比较差异有统计学意义(P<0.05),术前未化学治疗组COX-2蛋白表达及癌周LVD与淋巴结转移呈正相关(P<0.05).[结论]术前平阳霉素静脉化学治疗通过降低COX-2蛋白表达与抑制癌周淋巴管形成来减少淋巴结转移. OBJECTIVE To study the effects of preoperative intravenous chemotherapy with the pingyangmycin for COX-2 expression and lymphatic vessel density(LVD) in oral squamous cell carcinoma (OSCC). METHODS 45 paraffin block specimens of OSCC after operation were selected, in which 30 specimens were from simple surgery patients and 15 specimens from preoperative intravenous chemotherapy with pingyangmycin, and the expression of COX-2 and LVD marked with D2-40 in OSCC were detected by immunohistochemistry. RESULTS The positive expression rate of COX-2 and the average LVD around the carcinoma were significantly lower in preoperative intravenous chemotherapy group than in un-intravenous chemotherapy groups(P〈0.05), and the average LVD of tumor tissue was not obvious differences between two groups (P 〉 0.05). In un-intravenous chemotherapy groups, the LVD around the carcinoma was statistically significant difference between positive and negative groups of COX-2 expression, and COX-2 expression and the LVD around carcinoma were positive correlation with lymph node metastasis(P〈0.05). CONCLUSION Preoperative intravenous chemotherapy with the pingyangmycin could reduce lymph node metastasis through lowering COX-2 expression and inhibiting the lymphangiogenesis.
