前后路脊柱融合术治疗腰椎退行性变31例效果观察

Anterior and posterior spinal fusion in treatment of the degeneration of lumbar intervertebral disc(31 cases observation)

目的探讨前后路脊柱融合术治疗腰椎退行性变的临床效果。方法应用前后路脊柱融合术治疗腰椎退行性变31例,其中腰椎间盘突出症17例,腰椎失稳、椎管狭窄14例,观察治疗效果。结果术后29例腰痛消失,下肢麻木、疼痛逐渐好转,腰部稳定有力;2例患者术后患肢麻木感仍存在,残留有疼痛,影响行走功能。疗效评价优74.2%,良22.6%,差3.2%。术后随访腰椎间盘CT平扫,见椎体间相对运动消失,椎间隙较术前增高,椎间融合器与上下椎体融合,滑脱的椎体较术前复位明显,无移位并发症。结论前后路脊柱融合术治疗腰椎退行性变符合脊柱的生理负重,创伤小,近期效果满意。

Objective To explore the methods and clinical effect of anterior and posterior spinal fusion in treatment of the de- generation of lumbar intervertebral disc. Methods From 2006,1 to 2011,1,17 cases of lumbar intervertebral disc herniation and 14 cases of spinal canal stenosis were retrospectively analyzed after operation. Results Anterior and posterior spinal fusion with decom- pression of lumbar spinal canal can improve the lumbar compression symptoms in the treatment of lumbar degenerative diseases. Postop- erative CT examination shows the disappearance of the relative motion between the vertebrae, the intervertebral space is increased, and shows the fusion apparatus fused with fluctuation vertebral. Besides, the reset of the displaced vertebral body is obviously, no Displace- ment complications were observed. Conclusions This method conforms to the spinal physiologic weight-bearing with less surgery trau- ma. Clinical results are satisfactory.
