填塞时间对鼻内镜术后鼻腔出血及粘连的影响

Affectness of packing time on nasal bleeding and adhesions after endoscopic sinus surgery

目的比较不同抽出时间对鼻内镜鼻窦术后两大最常见并发症的影响。方法 2008年3月至2011年3月在我院行鼻内镜下鼻窦手术及鼻甲部分切除术186例患者,比较膨胀海绵填塞术后24h,48h,72h取出时出血情况及术后3周内鼻腔粘连情况。结果 24h取出出血率较48h及72h出血率高,鼻腔粘连情况24h与48h取出组相近,明显低于72h取出组,差异均具有统计学意义(P<0.01)。结论膨胀海绵48h为鼻窦及鼻甲部分切除术后患者最佳取出时间。鼻腔出血及粘连并发症最低。

Objective To compare different take ing time on two of the most common complications after function endo-scopic sinus surgery（FESS）. Methods A total of 186 patients who had undergone FESS from March 2008 to March 2011 were studied retrospectively. Comparing nasal bleeding and 3 weeks of adhesions when removed after packing 24h, 48h, 72h postoperative with expansion foam. Results Bleeding rate was higher in 24 hours when removed than 48h and 72h. Nasal adhesions were similar with 24h and 48h group when removed and significantly lower than the 72h group. There was statistically significant （ P 〈 0. 01 ）. Conclusions Expansion foam packing 48 hours is the best time of pa-tients after partial turbinate hepatectomy and sinus surgey. Complications of nasal bleeding and adhesion is the lowest.
