单孔与传统腹腔镜下肾囊肿去顶术的对照研究

A comparison of laparoendoscopic single-port decortication of renal cysts and the traditional laparoscopic decortication

目的比较单孔与传统腹腔镜下肾囊肿去顶减压术的治疗效果。方法单纯性肾囊肿患者共计40例。其中,男性22例,女性18例;平均年龄56.5岁(28～77岁)。其中15例采用单孔腹腔镜行肾囊肿去顶减压术,25例采用传统腹腔镜肾囊肿去顶减压术。比较两组手术时间、术中出血量、平均住院时间及腹膜后引流管停留时间等数据。结果全部肾囊肿去顶术均顺利完成,无中转开放,均无明显手术并发症。单孔组无病例需中转传统腹腔镜去顶术。单孔腹腔镜组和传统腹腔镜组手术时间分别为(58.3±20.4)和(52.1±18.0)min(P>0.05);出血量分别为(18.7±7.7)和(15.6±5.2)mL(P>0.05);住院时间分别为(6.0±1.0)和(6.9±1.4)d(P<0.05)。结论单孔与传统腹腔镜治疗单纯性肾囊肿具有相同的治疗效果,是可行有效的。

Abstract： [ Objective ] To compare the efficacy of laparoendoscopic single-port decortication of renal cysts with the traditional laparoscopic operation. [ Method ] Forty patients with simple renal cysts were operated on, including laparoendoscopie single-port deeortication of renal cysts in 15 cases and traditional laparoscopic decortieation of re- nal cysts in 25 cases. Thirty-two patients were male and 18 were female. The average age was 56.5 （range from 28 to 77）. The blood loss, operation time and the time of recovery were compared. [ Results ] The operation was per- formed successfully in all cases, no conversion to open surgery, and without any severe complications. None of the laparoendoscopie single-port operation was converted to traditional laparoscopic operation. In the laparoendoscopic single-port operation and the traditional laparoscopic operation, the mean operation time were （58.3±20.4） min, （52.1±18.0） min respectively （P 〉0.05）, and the mean estimated blood loss were （18.7±7.7） mL and （15.6±5.2） mL （P 〉0.05）, the mean hospital stay were （6.0±1.0） d and （6.9±1.4） d （P 〈0.05）. [ Conclusions ] Laparoendoscopic single- port decortication of renal cysts and traditional laparoscopic operation had the same clinical therapeutic effective- ness. It is effective and feasible.
