成角旋转快速牵引对腰椎间盘突出症的康复疗效研究 被引量:6

摘要 为观察成角旋转快速牵引对腰椎间盘突出症的康复疗效,将腰椎间盘脱出症患者分为A组126例,运用成角旋转快速牵引配合常规综合疗法治疗;B组126例,运用常规综合疗法治疗。结果:治疗后对比分析,A组疗效优于B组,痊愈天数较B组短,差异均具有非常显著性(P<0.01);对不同年龄及病程的患者,2组疗效差异均有显著性,但对A组的影响更显著(A组P<0.01,B组P<0.05)。>55岁或病程>1年的患者2组疗效比较差异无显著性(P>0.05)。同时探讨了成角旋转快速牵引的治疗原理。提示:成角旋转快速牵引可使突出物向椎间隙还纳,又可纠正小关节错位,增加侧隐窝及椎管容积,分离松解病变部位及神经根粘连压迫。该牵引治疗参数应设置为快速距5~7cm、旋转角±12~20°、下倾角15~25°有效且安全。 In order to observe the rehabilitation therapeutic effect of rapid rotation of angular rotation in the treatment of prolapse of lumbar intervertebral disc, the patients were divided into group A: n= 126, undergoing rapid traction of angular rotation combined with routine comprehensive therapyl and group B: n =126, receiving routine comprehensive therapy only. The results showed that the therapeutic effects in group A were better than in group B after treatment. The cured days in group A were shorter than in group B (P<0. 01). Different ages and duration in both two groups revealed different therapeutic effects, but more significant in group A than in group B (group A: P<0.01, group B= P<0.05). The therapeutic effects in the patients with age >55 or duration >1year in group A were not better than in group B (P>0.05). It was indicated that the rapid traction of angular rotation could make the protuberances come back into intervertebral space, correct the dislocation of small joints, increase the volume of lateral cryptae and vertebral canal, and dissociate and relax lesions and nervous root adhesive depressed regions. It was suggested that the traction parameters should be set up as: rapid distance: 5 to 7 cm, rotational angle: ±12 to 20°. down-inclination: 15 to 25°, which are effective and safe.
