重症胰腺炎肠内外营养的临床效果比较

Comparison of enteral and parenteral nutrition support treatment for severe acute pancreatitis patients

目的:探讨早期肠内营养与肠外营养对重症急性胰腺炎患者临床治疗效果。方法:将我院2010年1月～2011年12月收治的80例重症急性胰腺炎患者随机分成两组,对照组(TPN组)40例患者给予常规治疗,观察组(EN组)40例患者在使用常规治疗同时,在患者胃肠道功能刚恢复后早期放置鼻空肠营养管并过渡到肠内营养,通过比较治疗前后患者营养状况、临床效果、住院时间、住院费用及并发症发生情况。结果:观察组在营养状况方面,血淀粉酶及尿淀粉酶恢复时间、腹痛减轻时间、住院时间、住院费用、患者病死及并发症发生方面均显著优于对照组(P<0.05)。结论:肠内营养支持治疗重症急性胰腺炎的疗效确切,不良反应不少,可有效减少患者住院时间及医疗费,显著优于肠外营养支持治疗。

Objective. To study the early clinical therapeutic effects of enteral nutrition and paren- term nutrition in treating severe acute pancreatitis patients. Methods.A total of 80 patients with severe acute pancreatitis admitted from January 2010 to December 2011 were randomly divided into two groups in- cluding control group （TPN group） of 40 patients who were given conventional treatment and observation group （EN group） of 40 patients who had early placed nasal jejunum nutrition tube and transition to enter- al nutrition after recovering of gastrointestinal function based oin conventional treatment. Nutrition sta- tus, clinical effect, hospital stays, expenses and complications before and after treatment were observed and compared. Results： In EN group, nutritional status, blood amylase recovery time, urine amylase re- covery time, abdominal pain relief time, hospital stay length, hospital fees, mortality and complications were superior compared with TPN group （P〈 0.05）. Conclusions： The enteral nutrition support therapy shows exact curative effects on severe acute pancreatitis but inducing little adverse reaction, which can effectively shorten hospital stays, cut hospitalization expenses and is su!aerior to parenteral nutrition.
