液流荧光技术在AFP和CEA检测中的应用分析

摘要 目的探讨液流荧光技术在临床检测甲胎蛋白(AFP)和癌胚抗原(CEA)中的应用,并与电化学发光技术检测结果进行比较和评价。方法利用Luminex200和Cobase-411两台仪器检测30例健康对照者外周血清中AFP与CEA含量,计算两种方法的检测阈值,同时检测41例乙肝患者的AFP与CEA含量,分析两种检测结果的一致性及相关性。结果Luminex200与Cobase-41l的AFP与CEA的正常检测限分别为6.57,2.81ng/ml与4.19,2.38ng/ml。t值分别为2.799和1.485,P值分别为0.007和0.1429。以Cobase-411仪器作为参考仪器,Luminex200的AFP与CEA检测的灵敏度、特异度和准确度分别为94.4%,97.1%,95.8%与97.3%,88.2%,92.9%。两台仪器的相关性分析结果显示,AFP的相关系数达0.9以上,CEA的相关系数0.8左右。结论AFP和CEA在两种仪器的检测结果有各自的正常检测限,检测结果虽相关性较好,但不同仪器不同试剂不同溯源,必须使用各自的正常值范围,检验单必须注明仪器方法,检验结果的互认最好建立在同类仪器同种方法相同溯源的基础上。Luminex200以Cobase-41l为参考,具有较高的灵敏度、特异度和准硝度,而且该仪器具有高通量特性,可以根据检测项目的需求进行组合,在临床检测和科研上具有独特的优势,在实际操作上简便快捷,标本用量少,便于推广。 Objeciive To investigate the application of the liquid chip technology in clinical detection of the alpha-fetoprotein (AFP) and the carcinoembryonic antigen (CEA),then, comparing with the results detected by the electrochemiluminescence technology. Methods The seurm from 30 healthy donors were collected and the levels of AFP and CEA were detected by liquid chip technology (Luminex200) or electrochemiluminescence technology ( Cobase-411 ). The normal detection limits of each method were analysised. AFP and CEA levels of 41 patients were examined to show the correlation and comparability between two methods. Results AFP and CEA detection limit of Luminex200 and Cobas e-411 was 6.57 ng/ml,2.81 ng/ml and 4.19 ng/ml,2.38 ng/ml respectively. The paired t-test results of AFP and CEA of two methods were t= 2. 799 and L 485 ,P=0. 007 and 0. 142 9. Using Cobase e-411 as a control instrument, the sensitivity, specificity and accuracy of the AFP and CEA detected by Luminex200 were 94. 4%,97.1% ,95.8% and 97.3% ,88.2% ,92.9% respectively. The correla- tion analysis between two instruments showed that the correlation coefficient of AFP was more than 0.9,while that of CEA was about 0. 8. Conclusion The two systems have their own nomal detection limits of AFP and CEA, and they had a good correlation,but different instruments different reagent different reasoning. We must use their normal range, check list must indicate the instrument method, inspection results the mutual recognition of the best in the same instrument the same meth- ods established on the basis of the same source. Using Cobas e-411 system as a control,Luminex200 has higher sensitivity, specificity and accuracy. The instrument has high throughput characteristics, and may, according to the demand of the test i- tems are combined,in clinical testing and research has unique advantages,in actual practice was simple and direct, specimens usage,facilitate promotion.
