糖化清蛋白在糖尿病早期诊断中的应用价值 被引量:3

摘要 目的比较糖化清蛋白(GA)和糖化血红蛋白(HbAlc)在糖尿病早期诊断中的应用价值。方法选择324例疑似糖尿病患者,应用WHO诊断标准并通过绘制受试者工作特征曲线(ROC曲线),分析比较HbAlc和GA的诊断糖尿病最佳切点及敏感度和特异度。结果GA的ROC曲线最佳切点为17.02%,敏感度63.2%,特异度95.6%,曲线下面积0.856;HbAlc的ROC曲线最佳切点为6.48%,敏感度60.5%,特异度83.7%,曲线下面积0.772。结论GA可作为一个准确反映糖尿病患者近期血糖代谢情况的指标,对早期诊断糖尿病、指导糖尿病的治疗有较大意义。 Objective To compare the value of glycated albumin (GA) and glycated hemoglobin (HbAlc) when oral glucose tolerance test (OGTT) was used in diagnosis of diabetes mellitus (DM). Methods 324 cases with suspicious DM diagnosed were enrolled in this study. Receiver operating characteristic (ROC) curves were plotted for comparative analysis of the opti- mal cut point, sensitivity and specificity of GA and HbAlc in diagnosis of DM. Results The optimal cut point of ROC curves for GA in diagnosis of DM was 17.02% ,with a sensitivity of 63.2%,a specificity of 95.6% ,area of 0. 856 under the curve. The optimal cut point of ROC curves for HbAlc in diagnosis of DM was 6.48% ,with a sensitivity of 60.5% ,a speci- ficity of 83.7% ,area of 0. 772 under the curve. Conclusion GA can be used as an appropriate index which reflects glycemic variation in preceding short-term period and may be of greater importance in early diagnosis and therapy of DM.
