带锁髓内钉治疗长骨骨折及动力化结果48例分析

Analysis of interlocking intramedullary nail in treatment of 48 Patients with long bone fractures and dynamic results

目的探讨带锁髓内钉治疗四肢长骨骨折的效果及应用范围、相关并发症。方法回顾性分析应用带锁髓内钉治疗长骨骨折48例,观察其临床疗效及相关问题。结果术后平均骨折愈合时间28周,动力化安装平均愈合时间17周,静力化安装、动力化调整时间平均16周,平均愈合时间为动力化后21周。无一例死亡病例。开放骨折经髓内钉固定,未发生1例骨感染。随访平均19个月,膝踝关节均达到功能位,有伤口浅表感染等7项并发症,经处理痊愈。结论应用带锁髓内钉治疗长骨骨折愈合率高,并发症发生率低,是治疗长骨骨折的一种较为理想的内固定方法。

Objective To discuss the effect, application scope and related complications of interlocking intramedullary nail in the treatment of long bone fractures. Methods Took the retrospective analysis on the application of interlocking in- tramedullary nail in the treatment of 48 patients with long bone fractures for patients in the treatment group to observe the clinical effect and related issues. Results The mean post-operative fracture healing time was 28 weeks, mean healing time of dynamic installation was 17 weeks, mean time of static installation and dynamic adjustment was 16 weeks, and mean healing time after the dynamic treatment was 21 weeks. No patients died. For internal fixation by intramedullary pin in the open fracture, there were no patients suffered the bone infection. The mean follow-up time was 19 months. The knee and ankle joints all returned to the functional position and there were 7 complications as wound superficial infection, which were cured after the treatment. Conclusion Locked intramedullary nailing is one of the good methods of treating long bone fractures and it has a high union rate and a low complication rate.
