摘要 目的评估MRI动态增强扫描对甲状腺良恶性结节的鉴别及诊断价值。方法选择经FNC证实的甲状腺纤维腺瘤患者32例,甲状腺癌患者30例,均进行MRI平扫及动态增强扫描,比较不同病变的影像学特点,测量感兴趣区及周边组织信号值,比较不同病灶在不同时间的强化值,绘制TIC曲线,比较不同结节的TIC差异。结果平扫甲状腺腺瘤表现为T1WI呈等或低信号,T2WI多呈高信号,甲状腺癌表现为T1WI不均匀低信号,T2WI多为不规则高信号,增强扫描均见不同程度强化,MRI增强扫描甲状腺癌1、2 min强化率高于甲状腺腺瘤,TIC曲线甲状腺癌同甲状腺腺瘤存在明显差异,MRI增强扫描对甲状腺癌及甲状腺腺瘤的诊断率高于MRI平扫。结论甲状腺良恶性结节MRI动态增强扫描均具有特征性的影像学特点,TIC也存在显著的差异,MRI动态增强扫描有助于甲状腺结节的鉴别及定性诊断。 Objective To evaluate the value of dynamic enhanced MRI in diagnosis of benign and malignant thyroid nod- ules. Methods Thirty patients with thyroid cancer and 32 patients with thyroid gland fibroadenoma dignosized with FNC were selected and detected with common and dynamic enhanced MRI, imaging characteristic were contracted in different affection. Signal level were detected in ROI and circumambient tissue, reinforcement degree was ontracted in different af- fection. TIC curve was drawed and difference was contrasted in different affection. Results Thyroid gland fibroadenoma was identify signal and low signal in T1WI and high signal in T2WI, thyroid cancer was inhomogeneous low signal in T1WI and inhomogeneous high signal in T2WI,thyroid gland fibroadenoma and thyroid cancer was enhanced to some extent in dynamic enhanced MRI, reinforcement rate was higher in 1 rain and 2 min in thyroid cancer than in thyroid gland fi- broadenoma, TIC curve in thyroid gland fibroadenoma was discrepant between thyroid cancer and thyroid gland fibroade- noma. Conclusion There is characteristic imaging features in dynamic enhanced MRI in benign and malignant thyroid nodules, TIC curve was discrepant, dynamic enhanced MRI was helpful for identification and qualitative diagnosis in thy- roid nodule.
