basal-like乳腺癌临床病理学特点分析

摘要 目的探讨乳腺癌的分子学及免疫组织化学分型及基底细胞样(basal-like)乳腺癌的临床病理学特征。方法针对2000年1月至2010年12月于我院外科行乳腺癌根治疗性手术或乳腺癌保乳手术切除的标本中选出"三阴性"乳腺癌(ER阴性、PR阴性、HER2阴性)50例,采用ER、PR、HER2、CK5/6和EGFR组合免疫组织化学结果定义BLBC(ER-、PR-、HER2-、CK5/6+和/或EGFR+)和非BLBC(ER-、PR-、HER2-、CK5/6-和EGFR-)2个亚型。观察两组的临床及病理学特征。结果肿瘤大小、年龄、淋巴结是否转移、组织学类型、组织学分级、临床分期和局部复发情况在二种亚型中差异均无统计学意义,而月经状态、Ki-67及淋巴管浸润情况在各亚型中差异具有统计学意义。BLBC型乳腺癌绝经前比例较高,而Ki-67比率及淋巴管浸润亦均高于非BLBC型乳腺癌(P=0.009及P=0.008)。结论本组研究显示,在三阴性乳腺癌当中,与非BLBC比较,BLBC多发生于绝经后妇女,预后较差。
