临床药师药学问诊模式探讨

Exploration of pharmacy inquisition mode for clinical pharmacist

目的:探讨适应临床药学发展的药学问诊模式与方法。方法:以美国PDCA质量循环管理理论为指导,成立临床药师问诊工作小组,借鉴医学问诊,采取直接询问患者和角色模拟相结合的方式,制定药学问诊标准操作流程。结果:明确药学问诊的重要性,制定可涵盖患者疾病及用药信息、条理化、专业化、程序化的药学问诊标准操作流程。结论:药学问诊标准操作流程为临床药师提供了一种规范化的标准问诊模式,应将规范化的药学问诊渗透于患者的整个诊疗过程中,并在临床实践中不断检验和完善问诊流程,保障合理用药。

Objective： To explore and establish a standard mode of pharmacy inquisition which could accommodate the development of clinical pharmacy. Methods： Directed by the PDCA management theory, the inquisition workgroup of clinical pharmacist was set up. And then based on medical inquisition, standard operating flowsheet of pharmacy inquisition was made through directly inquiring patients and simulation role inquisition. Results： The significance of pharmacy inquisition was identified. The categorization, specialization and sequencing operating flowsheet of pharmacy inquisition containing the patients＇ information of disease and medication was established. Conclusion： The standard operating flowsheet of pharmacy inquisition could provide a normalization mode of inquisition for clinical pharmacist. Clinical pharmacist should perform it during the whole process of the patients＇ treatment and promote it in the clinical practice in order to ensure rational administration.
