大龄儿童股骨干骨折的治疗

Treatment of femoral shaft fractures in older children

目的 总结大龄儿童股骨干骨折内固定治疗经验。方法 30例上段或中上段 1/ 3骨折采用髓内针固定 ,48例中下 1/ 3及下段骨折采用钢板固定 ,4例大斜面及螺旋形骨折采用钢丝 +螺钉固定。结果 74例获 6个月～ 11年随访 ,无骨折延迟愈合及不愈合。 5 6例双下肢等长 ,18例患肢过度生长 ,关节功能、步态正常。并发症 :迟发感染 1例 ,再骨折 2例。结论 采用内固定复位满意 ,固定可靠 ,并发症少 ,是治疗大龄儿童股骨干骨折的理想方法之一。

Objective To summarize the experience on the treatment of older childrens femoral shaft fracture by intermal fixation. Methods 30 cases of femoral fracture in the upper or 1/3 mid upper segment were fixed with intramedullary nails,48 cases of femorel fracture in the middle inferior segment with steel plate, and 4 cases of incline or spiral fracture with wires and screws. Results 74 cases were followed up for 6 months to 11 years.There were nonunion and delayed union.56 cases were the same length in the lower limbs,the wounded limbs of 18 cases grew excessively. The knees functions and steps were normal. The complications included 2 cases of re fracture,and 1 case of late infection. Condusion Satisfactory effects are obtained in the reduction by internal fixation, which fixs firmly and produces fewer complications. The procedure is one of the ideal choices to treat older childrens femoral shaft fracture.
