创伤性颈动脉海绵窦瘘的介入治疗

Interventional treatment for traumatic carotid-cavernous fistula

目的探讨介入治疗对创伤性颈动脉海绵窦瘘的临床疗效和治疗安全性。方法对本院2007年1月～2011年1月收治的创伤性颈动脉海绵窦瘘患者10例进行可脱性球囊介入治疗,观察患者的成功率和治疗后症状的改善程度。结果 10例患者接受介入治疗后,7例颈内动脉恢复通畅,占70%,眼球突出、充血、眼球运动障碍等临床症状随之消失;1例因就诊时间较迟遗留左眼单盲,其余2例患者的临床症状也有不同程度改善。结论可脱性球囊介入治疗是创伤性颈动脉海绵窦瘘的理想疗法,效果好、安全性高,临床上建议进一步推广运用。

Objective To discuss the clinical effects and therapeutic safety of interventional treatment for traumatic carotid-cavernous fistula （TCCF）.Methods Ten cases of patients with TCCF in our hospital from January 2007 to January 2011 were all given interventional treatment with detachable balloon.The patients＇ success rate and degree of symptomatic improvement were observed.Results After interventional treatment,internal carotid artery recovered patency in 7 cases,accounting for 70%,and their clinical symptoms of eyeball outstanding,congestion and eye movement obstacles disappeared;1 case left over left eye single-blind due to late visiting time;the clinical symptoms of remaining 2 cases also had different levels improvement.Conclusion Interventional treatment with detachable balloon is an ideal therapy for TCCF,which has good effect and high safety.It is worthy of further promotion in clinic.
