肾移植后肺部真菌感染的CT表现

CT imaging features of pulmonary fungal infection in kidney transplantation patients

目的探讨肾移植后肺部真菌感染的CT表现。方法回顾性分析经实验室或病理证实的58例肺部真菌感染的CT影像表现。结果双肺弥漫、散在多发41例;左肺多发7例,左肺下叶单发3例,左肺上叶单发1例;右肺多发3例,右肺下叶单发2例,右肺上叶单发1例。真菌感染的肺部CT表现比较复杂,多种征象同时出现,34例表现为双肺弥漫分布的磨玻璃影;12例表现为肺内实变影;12例表现为结节或肿块样病变,大部沿血管束走行;6例出现双肺粟粒结节。结论肾移植后肺真菌感染影像学表现多种多样,病变常多部位发生,多种影像表现同时出现,病灶形态变化快。CT检查对肾移植后肺部真菌感染的早期诊断具有重要价值。

Objective To discuss the CT imaging features of pulmonary fungal infection in kidney transplantation patients.Methods The CT imaging features of 58 cases with pulmonary fungal infection proved by pathology were reviewed retrospectively.Results Forty one cases showed scattered or multiple lesion in bilateral pulmonary,7 cases showed multiple lesion in left lung,3 cases showed single lesion in lower lobe of left lung,1 case showed single lesion in upper lobe of left lung,3 cases showed multiple lesion in right lung,2 cases showed single lesion in lower lobe of right lung,and 1 case showed single lesion in upper lobe of right lung.CT manifestations of pulmonary fungal infections were fairly complex with simultaneous appearance of various signs.34 cases showed diffuse distribution of ground-glass opacities;12 cases showed pulmonary opacities;12 cases showed nodular or mass-like lesions,most of the vascular bundle to go along the line;6 cases showed military nodules in bilateral lung.Conclusion The imaging of pulmonary fungal infections after kidney transplantation is varied,multi-site disease often occurs,the performance of multiple images appears simultaneously,lesion morphology changes quickly.CT scan has a great value for the early diagnosis of pulmonary fungal infections in kidney transplantation patients.
