国内不同级别医院消化内镜数量调查与分析:169家医院调查报告

Investigation of the volume of gastrointestinal endoscopes in hospitals at various levels: An investigation from 169 hospitals

目的调查分析我国不同级别医院消化内镜数量及每年完成消化内镜操作数量等。方法以国内不同省份的各级医院消化内镜单元(室)为调查对象;采用电子邮件或电话问卷方式进行调查分析。结果 2010年5月至2010年11月,共有169家医院进入本次调查,分布于28个省,占国内大陆省份的90.3%(28/31),其中147家医院可开展消化内镜操作,占87.0%(147/169);95.7%(44/46)的三级医院消化内镜操作数≥5000例次/年,明显高于二级医院(12.1%,11/91),且差异有统计学意义(χ2=88.8,P<0.05);一级医院消化内镜操作数均≤3000例次/年;147家开展消化内镜的医院拥有消化内镜数平均为(9.3±3.2)条/单元;其中三级医院为(23.4±6.5)条/单元,二级医院为(4.6±1.4)条/单元,一级医院为(1.3±0.5)条/单元;83.6%(123/147)的医院使用Olympus消化内镜。结论过去10年来国内医院消化内镜平均配备量明显提高,但三级大医院对内镜的配备量及每年完成消化内镜操作量仍远远超过中小医院;Olympus是目前国内消化内镜的主要供应者。

Objective To investigate the volume of gastrointestinal （GI） endoscopes in different scale hospitals in China's Mainland. Methods The survey was conducted by a questionnaire sent through Email or telephone to the hospital-based GI endoscopy units, including three levels of hospitals （Third-Grade Hospital Classification in China）. Results From May 2010 to November 2010, 169/279 （60.6%） units were enrolled, which covered 28 provinces （90.3%） in China's Mainland. Among the 169 hospitals, 147 （87.0% , 147/169） hospitals perform GI endoscopy. The totall number of GI endoscopy procedures ≥5000/year were found in 95.7% （44/46） of the tertiary hospitals, significantly higher than that in the second-level hospitals（ 12.1%, 11/91 ）（X2 = 88.8 ,P 〈 0.05 ）, and tall the first-level hospitals were less 3000 procedures/year. The average number of GI endoscopes in 147 hospitals was （9.3 ±3.2 ） /unit. Furthermore, the average number of GI endoscopes in the tertiary hospitals was （23.4 ± 6.5 ）/unit, （4.6 ± 1.4）/unit in the second-level hospitals, and （ 1.3 ± 0.5 ）/unit in the first-level hospitals. GI endoscopes manufactured by Olympus was found in 83.6% of the 147 hospitals. Conclusions The average number of GI endoscopes in different scale hospitals is substantially elevated in the past decade in China's Mainland, however, far more GI endoscopes and endoscopic procedures are predominantly confined to the tertiary hospitals instead of medium-small ones. Olympus is the major provider of GI endoscopes in China's Mainland.
