超声心动图引导球囊房间隔造口术治疗新生儿重症先天性心脏病 被引量:3

BALLOON ATRIAL SEPTOSTOMY UNDER DOPPLER ECHOCARDIOGRAPHIC CONTROL FOR THE TREATMENT OF CRITICAL NEWBORN WITH CONGENITAL HEART DISEASE

摘要 目的、方法 :采用 UM4型超声心动图仪床边监测引导球囊房间隔造口术(BAS)姑息治疗新生儿重症先天性心脏病 10例 ,其中完全性大动脉转位 5例 ,完全性肺静脉畸形引流 3例 ,解剖纠正型大动脉转位和左心发育不良并肺静脉引流异常各 1例。结果 :10例中 8例造口成功 ,效果满意。超声测量房间隔缺损口径 4.5~10 mm,平均 7.5 mm;多普勒测穿隔血流速度由 1.87± 0 .2 m/ sel降至 0 .71± 0 .2 m/eel;压力阶差由 12 .70± 3.0降至 2 .0 6± 1.46 mm Hg(P <0 .0 1)。 5例完全性大动脉转位血氧饱和度由 5 3.6± 15 .96 %升至 78.0± 12 .5 5 % (P <0 .0 1)。2例不成功因合并卵圆孔早闭及心室反位罕见畸形引起。结论 :BAS在超声引导下更加简便易行 ,安全可靠 ,对改善依赖卵圆孔开放维持生命的先心病患儿临床症状。 Balloon atrial septostomy (BAS) was undertaken under Doppler echocarediography guidance in 10 conscutive newborns and infants.Their median age was 21.7 days.5 patients had transposition of the great arteries (TGA),3 had total anomalous pulmonary venous connection,the other two had anatomical corrected TGA and hypoplastic left heart syndrome respectively.The procedure was successful in 8 cases,the median size of the created interatrial opening was 7.5mm,the pressure gradiant between right and left atrial decreased from 12 70±3 0mmHg to 2.06±1 46mmHg(p<0 01)。and the arterial oxygen saturation in 5 TGA cases was significantly increased from 53.6±15 96% to 78.0±12.55%(<0 01) BAS under echocardiographic imaging control is an efficient,safe and easy procedure.(Original article in page 103)
