低剂量对比剂在头颈部3D CE-MRA中的应用研究 被引量:1

摘要 目的探讨使用小剂量对比剂行头颈部动脉三维对比增强磁共振血管成像(3D CE-MRA0的可行性。方法 100例临床怀疑颅颈部血管病变的患者分成A、B两组,使用1.5T磁共振扫描仪行颅颈部动脉3D CE-MRA,A/B组使用剂量分别为0.15mmol/kg和0.30mmol/kg,比较两组患者图像最大密度投影(MIP)图像质量及原始增强图像颈总动脉、颈内动脉及大脑中动脉的信号强度(SI)、信噪比(SNR)和对比噪声比(CNR)。结果由两位医师在不知临床资料和对比剂用量的情况下共同评价MIP图像质量,发现A、B两组的MIP图像质量均符合诊断要求,差异无统计学意义(P>0.05),A组的静脉污染比B组略轻,但差异无统计学意义,A组显示分支血管的级数略低于B组,但差异亦无统计学意义;A、B两组颈总动脉的SI、SNR、CNR及颈内动脉的SI差异无统计学意义,A组颈内动脉SNR、CNR及大脑中动脉的SI、SNR、CNR均略低于B组,差异有统计学意义(P<0.05)。结论采用1.5T磁共振仪行头颈部3D CE-MRA,使用0.15mmol/kg的钆对比剂可以获得良好的图像质量并满足临床诊断。 Objective To evaluate the feasibility of low dose three-dimensional contrast-enhanced MR angiography(3D CE-MRA) of cranial carotid arteries.Methods 100 cases clinical doubt cranial vascular lesions neck of A and B patients into two groups,use 1.5 T magnetic resonance scanner line cranial neck artery 3 D CE-MRA,A/B group the dose were 0.15 and 0.30 mmol/kg mmol/kg,compared with two groups of patients image most density projection(MIP) image quality and original enhance image common carotid artery,the internal carotid artery and of the middle cerebral artery signal strength(SI),signal-to-noise ratio(SNR) and the contrast between noise ratio(CNR).Results A,B two group of MIP image quality which conform to the diagnosis requirements,the difference was not statistically significant(P 0.05) group A vein of pollution than group B slightly light,but the difference was not statistically significant,A group of display the blood vessels of the branch series slightly lower than group B,but have no statistically significant differences;A and B two groups of common carotid artery SI,SNR,CNR and internal carotid artery is not statistically significant differences between the SI,A group of internal carotid artery SNR,CNR and of the middle cerebral artery SI,SNR,CNR are slightly lower than group B,the difference was statistically significant(P 0.05).Conclusion The 1.5 T magnetic resonance instrument do head and neck 3 D CE-MRA,use 0.15 mmol/kg gadolinium contrast agents can obtain good image quality and meet the clinical diagnosis.
