摘要 目的研究颅脑损伤后凝血功能的变化及临床意义。方法对本院2009年9月至2010年9月收治的51例单纯性颅脑损伤患者及23例健康对照组进行研究,监测其伤后24h内体内凝血酶原时间(PT)、凝血酶时间(TT)、部分凝血活酶时间(APTT)、纤维蛋白原(FIB)及血小板(PLT)的变化。结果将单纯性颅脑损伤患者监测数据与健康对照组患者以及中、轻型颅脑损伤组患者进行对比,重型颅脑损伤组患者伤后24h内体内血浆PT、APTT、TT、FIB和PLT分别为(16.565±3.563),(29.755±4.731),(18.009±0.627),(2.283±1.211),(142.900±30.675),其中PT、APTT、TT明显延长,差异有显著性(P<0.05);FIB和PLT均减低,但无统计学意义。结论重型颅脑损伤后可发生凝血功能异常。 Objective To study coagulation changes and clinical characteristics after brain injury.Methods The prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB) and platelet count(PLT) were detected in 51 brain injured patients within 24 hours after traumas,same measurements were done in 23 normal control patients.Results The PT,APTT,TT,FIB and PLT in severe brain(GCS≤8) injury group were(16.565±3.563),(29.755±4.731),(18.009±0.627),(2.283±1.211),(142.900±30.675).Compared with normal contral group and the group of(GCS8),PT,APTT and TT were lengthed(P0.05),FIB and PLT were reduced(P0.05).Conclusion Severe brain injury can cause coagulation abnormalitics.
