河南省2009～2011年麻疹监测系统运转情况分析

目的分析河南省2009年以来麻疹监测系统的运转情况。方法采用描述流行病学方法 ,对2009～2011年通过疾病监测系统报告的麻疹病例和监测指标进行分析评价。结果河南省2009～2011年分别报告麻疹确诊病例9203例、2250例和169例,<8月龄病例分别占病例总数的20.81%、27.78%和34.32%,≥15岁病例分别占病例总数的11.19%、13.24%和13.61%;8月龄～14岁病例中,有免疫史者所占比例分别是36.03%、41.91%和51.72%。近三年麻疹排除病例发病率分别为1.35/10万、0.94/10万、0.71/10万,麻疹疑似病例48h内完整调查率为90.45%、92.05%、97.97%,散发病例血标本采集率分别为63.27%、78.57%、96.74%,实验室结果 7d内反馈率分别为61.16%、78.53%、99.14%。结论河南省2009～2011年麻疹病例数和发病率显著降低的同时,小年龄组病例和成人病例所占比例增加,应采取针对性措施,进一步降低麻疹发病率。建议通过加强预防接种单位管理、提高常规免疫工作质量来提高含麻疹成分疫苗的接种率,尤其是第一针及时接种率;积极推动育龄期妇女接种含麻疹成分疫苗;同时做好麻疹病例的精细化管理,防止传染源扩散。自2009年麻疹专病报告管理系统使用以来,近三年麻疹监测系统的监测水平逐步提高,2011年监测系统的及时性和特异性指标都大于95%。

Objective To evaluate the running status of measles surveillance system （MSS） of Henan province. Methods We reviewed the case investigation data of measles -like Illnesses （MLIs）.reported through MSS during 2009 and 2011 .Performance indicators of the MSS were analyzed. Results Totally 9203 cases, 2250 cases, 169 cases were reported through MSS in Henan province in 2009,2010, 2011.Of which cases younger than 5 years old accounted for 68.11%, 76.53%, 82.74% of all cases. The morbidity of Ruled out cases is 1.35/100000,0.94/100000,0.71/100000, 90.45%, 92.05%, 97.97% of all the Suspected measles cases were surveyed within 48 hours, and 63.27%, 78.57%, 96.74% of Suspected measles cases were collected serological specimens; 61.16%, 78.53%, 99.14% of all the Serological specimens were reported laboratory test result within 7 days. Conclusion The percentage of cases younger than 5 years is high, and we suggest improve the Immunization coverage rate by enhancing the management of vaccination clinic .The timeliness, specificity and sensibility of MSS in Henan is better during 2009 and 2011.
