腹腔镜在诊断和治疗不孕症方面的价值探讨

The value explore of the application of laparoscopy in diagnosis and treatment of infertility

目的研究并探讨应用腹腔镜诊断和治疗不孕症的临床价值。方法选择本院2010年1月～2011年5月行腹腔镜诊治的不孕症患者400例,并对其临床资料进行回顾性分析。结果手术成功完成399例,1例因重度盆腔粘连中转开腹。术后患者多数无不适症状,少部分有肩背部稳痛不适和轻度腹胀,经对症处理后缓解。400例发现有盆腔器质性疾病,其中,盆腔粘连、慢性盆腔炎210例,多囊卵巢60例,子宫内膜异位症70例,子宫畸形6例,盆腔结核21例,其它33例。术后行1年随访,妊娠率达35%。结论腹腔镜手术具有出血小、微创、降低患者痛苦程度、对患者干扰小、明显缩短住院时间、加快术后恢复的优点,可在各种妇科疾病中安全有效的应用。以最大限度的提高手术成功率,降低并发症的发生,消除盆腔病变,达到治疗不孕的目的 。

Objective To study and explore the clinical value of the application of laparoscopy in diagnosis and treatment of infertility.Methods The clinical data of 400 patients with infertility treated with laparoscopic diagnosis and treatment in our hospital from January 2010 to May 2011 were retrospectively analyzed.Results The operation of 399 cases was successfully performed,1 case was converted to laparotomy for severe pelvic adhesions.The majority of patients had no dis comfort symptoms in postoperative,few had the instability pain and discomfort of shoulder and mild abdominal distention,and were remission after symptomatic treatment.400 patients were found to have pelvic implement qualitative disease,in cluding 210 cases of pelvic adhesions and chronic pelvic inflammation,60 cases of polycystic ovary,70 cases of en dometriosis,6 cases of uterine malformation,21 cases of pelvic tuberculosis,33 cases of the other.The follow-up was 1 year in postoperative,the pregnancy rate was 35%.Conclusion Laparoscopic operation has the advantages of little bleeding,minimally invasive,reducing the pain of patients,little interference for patients,shortening the hospitalization time,fasting postoperative recovery,and can be applied in various gynecological diseases with the safe and effective,to increase the successful rate of operation with maximize,reduce the incidence of complications,eliminate pelvic lesions,and reach the purpose of treating infertility.
