多功能取环钳与传统取环钩的临床应用对比研究

Clinical comparative study of multifunctional clamp and traditional hook for removal of IUD

目的探讨多功能取环钳和传统取环钩在宫内节育器放取中的应用效果。方法 2009年1月～2010年12月在我院行宫内节育器(IUD)取环的妇女中选择600例,随机分为研究组和对照组,各300例,研究组妇女采用多功能IUD取环钳,对照组采用传统的取环钩,比较两组取环时间、患者疼痛情况、术中出血量。结果两组取环妇女年龄、孕产次、置器时间差异无统计学意义(P>0.05)。研究组取环疼痛评分、取器时间、术中出血量分别为(2.46±0.32)分、(2.55±0.57)min、(1.68±0.46)mL,与对照组相应指标比较[(5.87±0.44)分、(3.19±0.63)min、(4.33±0.45)mL],差异有统计学意义(P<0.05)。并且研究组取环顺利者明显多余对照组,取环困难、失败者明显少于对照组,差异有统计学意义(P<0.05)。研究组子宫壁损伤及节育器断裂发生率明显少于对照组(P<0.05)。结论在节育器取出中采用多功能取环钳,可降低取器难度,提高取器准确性,减少手术对受术者的创伤,具有先进性和实用性,值得进一步推广。

Objective To investigate the application of multi-function clamp and traditional hook for removal of IUD.Methods 600 cases of women who take IUD removal in our hospital from January 2009 to December 2010 were randomly divided into study group and control group,300 cases of women in the study group used multi-functional IUD remove clamp,and control the group used the traditional extractor hook,IUD removal time,the pain situation,blood loss between the two groups were compared.Results The age,pregnant and production time,IUD taken time of two groups were not sig-nificantly different.Pain scores,IUD removal time,blood loss of study group [（2.46 ±0.32） scores,（2.55±0.57） min,（1.68± 0.46） mL] were less than control group [（5.87±0.44） scores,（3.19±0.63） min,（4.33±0.45） mL],the difference was statistically significant（P0.05）.And patients who removed IUD smoothly of study group were significantly more than the control group,patients who removed IUD difficultly were less than the control group,the difference was statistically significant（P0.05）.The uterine wall damage and the IUD fracture incidence was significantly less than the control group（P0.05）.Conclusion Multifunctional IUD removal clamp can reduce the extractor difficulty,improve the accuracy of the extractor,and reduce surgical trauma by the surgeon.It has advancement and practicability and worthy of further promotion.
