脑脊液置换术治疗蛛网膜下隙出血100例疗效观察

目的探讨脑脊液置换术治疗蛛网膜下隙出血的疗效。方法湛江中心人民医院收治的蛛网膜下隙出血患者,根据其治疗方式不同分为观察组和对照组。观察组采用脑脊液置换术,对照组采用传统保守治疗,观察两组疗效。结果观察组头痛持续时间、脑膜刺激征持续时间、CSF恢复正常时间等均明显少于对照组(P<0.05),治疗后Glasgow预后评分明显优于对照组(P<0.05),CV(S脑血管痉挛)、脑积水和再出血的发生率均明显低于对照组(P<0.05)。结论脑脊液置换术可明显改善SAH患者的临床症状和体征,而且并发症发生率少,操作简单、安全可靠,值得推广。

Objective To investigate Cerebrospinal fluid replacement surgery in the treatment of subarachnoid hemorrhage. Methods Patients with subarachnoid hemorrhage in our hospital were divided into two groups according to their treatment. The replacement of cerebrospinal fluid was used in the observation group and the traditional conservative treatment were used in the control group, and then observed the efficacy of treatment. Results The duration of headache, meningeal irritation sign and the CSF recovery time in the observation group were significantly less than the control group （P〈0.05）. The prognostic scoring of Glasgow after treatment in the observation group were significantly better than the control group （P〈0.05）. The incidence of CVS （cerebrovascular spasm）, hydrocephalus and rebleeding was significantly lower than the control group （P〈0.05）. Conclusion CSF replacement surgery can significantly improve the clinical symptoms and signs of SAH patients, and for its less complication rate, simple operation, safe and reliable, it is worth to promoting.
