奥氮平治疗慢性阻塞性肺病呼吸衰竭伴精神兴奋症状临床分析

Clinical analysis of olanzapine in the treatment of respiratory failure with mental excitement symptom in patients with chronic obstructive pulmonary disease

目的探讨奥氮平对慢性阻塞性肺病(COPD)呼吸衰竭伴精神兴奋症状的治疗效果和安全性。方法 48例COPD呼吸衰竭伴精神兴奋症状患者随机分成治疗组及对照组,对照组仅行常规抗感染、平喘、化痰等药物治疗,治疗组在常规药物治疗的基础上奥氮平治疗。比较治疗前、治疗后3 d两组精神兴奋症状改善及血气分析变化。结果治疗组总有效率显著高于对照组(P<0.05),治疗后3天两组血气分析(PaO2、PaCO2)比较无明显差异。结论奥氮平可以安全地用于COPD呼吸衰竭伴精神兴奋症状患者的治疗。

Objective To explore the clinical efficacy and safety of olanzapine in the treat- ment of respiratory failure with mental excitement symptom in patients with chronic obstructive pul- monary disease （COPD）. Methods Forty - eight COPD patients with respiratory failure with mental excitement symptom were randomly divided into control group （n = 23 ） and treatment group （n = 25）. The control group received conventional drug therapy including anti- infection, relieving airway spasm and reducing phlegm, etc. The treatment group received olanzapine on the basis of conventional drug therapy. The changes of mental excitement symptom and blood gas analy- sis were compared before treatment and 3 days after treatment in two groups. Results The total ef- ficacy rate in the treatment group was significantly higher than that in the control group （P 〈 0. 05）, blood gas analysis （PaO2, PaCO2） of the two groups showed no obvious difference 3 days after treatment. Conclusion Olanzapine can be used safely in the treatment of respiratory failure with mental excitement symptom in COPD patients.
