216例围手术期预防使用抗菌药物调查分析

Investigation and analysis of 216 surgical cases with prophylactic use of antimicrobial drugs

目的调查围手术期预防性使用抗菌药物的现状,推进《抗菌药物临床应用管理办法》实施工作进程。方法抽取2011年5月～2012年4月我院围手术期预防性使用抗菌药物病历216份,按照卫生部2004～2012年5月制定的有关围手术期预防性使用抗菌药物的指标,综合评价其用药合理性。结果 3类切口包含21种手术,使用5类抗菌药物,其中头孢菌素类居多,使用二联抗菌药物的,以头孢菌素类+硝咪唑类居多;围手术期预防性抗菌药物使用率为93.06%,其中Ⅰ类切口手术抗菌药物使用率为88.81%;围手术期预防性使用抗菌药物的合理率为48.61%。结论围手术期预防性使用抗菌药物存在一定的不合理问题,与卫生部规定指标差距较大。有待进一步加强抗菌药物临床应用知识的培训与教育工作。

Objective To investigate the status of preoperative prophylactic use of antimicrobial drugs in our hospi- tal, and promote the clinical use of antibiotics measures for the implementation process. Methods We collected 216 prophylactic uses of antimicrobial drugs medical records from May, 2011 to April, 2012, and then conducted a compre- hensive evaluation of drug rationality related to preoperative prophylaetie use of antimierohial agents indicators developed in the Ministry of Health from 2004 to May Z012. Re.suits There were three types of incisions that contains 21 kinds of operation among which five antimicrobial drugs used, mostly cephalosporin, and those combined antibacterial drugs, mostly cephalosporin and imidazole nitrate. The use of preoperative prophylactic antibiotics was 93.06 %. Class I incision antimicrobial drug use was 88.81%, and a reasonable rate of preoperative prophylactic use of antimicrobial drugs was 48. 61%. Conclusion There is a certain irrational gap of preoperative prophylactic use of antimicrobial drugs with the Minis- try of Health indicators. Antibacterial drug clinical application of the knowledge, training and education are needed to be further strengthened.
