手术治疗不稳定型骨盆骨折的近期疗效 被引量:2

摘要 目的探讨不稳定型骨盆骨折的手术治疗方法及近期疗效。方法回顾性分析2004年2月至2011年2月期间收治且获得随访的113例新鲜不稳定型骨盆骨折患者资料,男75例,女38例;中位年龄为41岁(12-73岁)。骨折按Tile分型:B型49例,其中B1型24例,B2型14例,B3型11例;C型64例,其中C1型41例,C2型12例,C3型11例。56例患者存在合并伤。所有患者均采用手术治疗,根据骨折类型不同采用单纯前环固定24例,单纯后环固定36例,前后环联合固定53例。末次随访时根据Majeed功能评分和Harris髋关节评分标准评定患者的临床疗效。结果 113例患者术后获8-60个月(平均24个月)随访。末次随访时采用Majeed功能评分标准评定疗效:平均评分为88.2分(48-100分),其中优76例,良22例,可12例,差3例,优良率为86.7%;31例合并髋臼骨折患者优良率为77.4%(24/31),18例合并股骨骨折患者优良率为77.8%(14/18)。末次随访时采用Harris髋关节评分标准评定疗效:平均评分为89.5分(46-100分),其中优78例,良24例,可6例,差5例,优良率为90.3%。12例合并腰骶丛神经损伤患者中,6例遗留有不同程度足踝背伸受限。结论选择合适手术方法治疗不稳定型骨盆骨折可取得良好疗效。
