网膜素与2型糖尿病非酒精性脂肪肝病的病例对照研究 被引量:4

A case-control study on the correlation between the level of omentin and non-alcoholic fatty liver disease in type 2 diabetes mellitus

摘要 目的分析网膜素与T2DM非酒精性脂肪肝病(NAFLD)发生的关系。方法采用ELISA测定190例新诊断T2DM患者(不合并NAFLD 102例为A组;合并NAFLD 88例为B组)空腹血浆网膜素水平,比较2组各代谢参数,并对各变量进行相关分析和Logistic回归分析。结果 B组空腹血浆网膜素水平显著低于A组[(53.67±21.84)ng/ml vs(70.74±21.17)ng/ml](P<0.05)。在T2DM患者中血浆网膜素水平与FIns、胰岛素抵抗指数(HOMA-IR)、WC、FPG、2hPG呈负相关(P<0.05),与HDL-C呈正相关(P<0.05)。Logistic回归分析结果显示网膜素作为独立因素影响NAFLD。由受试者工作特征(ROC)曲线得到网膜素预测脂肪肝的大致风险值为40ng/ml。结论网膜素可能影响T2DM患者NAFLD的发生发展。 Objective To investigate the relationship between the level of plasma omentin and onset of non-alcoholic fatty liver disease (NAFLD) in type 2 diabetes mellitus. Methods The level of fasting plasma omentin of incipient T2DM patients (102 without NAFLD, group A; 88 with NAFLD, group B) was assayed by enzyme-linked immunosorbent assay (ELISA). The metabolic indexes of the two groups were compared, the correlation of variables was analyzed, and the logistic regression analysis was made in the trial. Results The level of fasting plasma omentin was significantly lower in group B than in group A E(53. 67±21.84) ng/ml vs (70. 74±21. 17) ng/ml] (P〈0. 05). The correlation analysis of variables showed that the level of fasting plasma omentin was in a negative correlation with Fins, HOMA-IR, WC, FPG, and 2 hPG (P〈0. 05) and in a positive correlation with HDL-C (P〈0. 05). Logistic regression analysis indicated that omentin was an independent factor that influenced NAFLD. The risk value of NAFLD of omentin prediction obtained from ROC is 40 ng/ml. Conclusion Omentin may contribute to the development of NAFLD in T2DM.
