摘要
目的探讨无继发感染的湿疹患者皮肤菌群分布及对抗菌药物的敏感性。方法选取2011年1月-2012年3月在皮肤科诊断为泛发型湿疹112例住院患者作为研究对象,采用VITEK-1I全自动微生物分析仪进行患者皮损部位分泌物菌群鉴定,纸片扩散片(K-B)法进行药物敏感试验分析。结果 112例患者其中64例患者皮损分泌物检出感染菌株,检出率61.61%,其中5例患者同时存在2种细菌多重感染,多重感染率7.81%,共检出感染菌株69株,其中检出金黄色葡萄球菌35株占50.71%,表皮葡萄球菌18株占26.09%,产色葡萄球菌6株占8.70%,人葡萄球菌6株占8.70%,松鼠葡萄球菌2株占2.90,华纳氏葡萄球菌1株占1.45%,生葡萄球菌1株占1.45%;金黄色葡萄球菌、表皮葡萄球菌、产色葡萄球菌、人葡萄球菌普遍对左氧氟沙星、庆大霉素的敏感率>50.00%,对万古霉素、利福平的敏感率>90.00%。结论无继发感染的泛发型湿疹患者皮肤带菌率高,其中以金黄色葡萄球菌及表皮葡萄球菌为主,药敏试验对显示不同感染菌对药物敏感性各不相同,差异性较大。
OBJECTIVE To explore the distribution of the skin flora in the hairstyle eczema patients without secondary infections and the drug susceptibility to antibiotics.METHODS A total of 112 hospitalized patients,confirmed as hairstyle eczema in dermatology department from Jan 20011 to Mar 2012,were selected as the study objects,the automatic VITEK II microbial analyzer was employed to identify the flora from the lesional secretion,the drug susceptibility testing was performed by using the disk diffusion(K-B) method.RESULTS Totally 64 of 112 patients were detected the infected flora from the lesional skin with the detection rate of 61.61%,of which 5 patients were detected with two species of bacteria simultaneously with the multiple infection rate of 7.81%.A total of 69 strains of pathogens were isolated,including 35 strains of Staphylococcus aureus(50.71%),18 strains of Staphylococcus epidermidis(26.09%),6 strains of Staphylococcus chromogenic(8.70%),6 strains of Staphylococcus homis(8.70%),2 strains of Staphylococcus squirrel(2.90),1 strain of Staphylococcus warner(1.45%),and 1 strain of Staphylococcus born(1.45%);the drug susceptibility rates of S.epidermidis,S.aureus,and S.homis to levofloxacin and gentamicin were higher than 50.00%,the drug susceptibility rates to vancomycin and rifampicin were higher than 90.00%.CONCLUSION The skin flora carrier rate is high in the hairstyle eczema patients without secondary infections,S.aureus and S.epidermidis are the predominant species,the drug susceptibility testing result indicates that the species of the pathogens vary in the drug susceptibility,the difference is significant.
