脉复生合剂治疗糖尿病足的临床疗效观察

Clinical Observation of Maifusheng Mixture for Treatment of Diabetic Foot

【目的】观察脉复生合剂治疗各期糖尿病足(DF)患者的临床疗效。【方法】将82例患者随机分为治疗组42例和对照组40例。2组均给予基础治疗(糖尿病饮食、胰岛素强化治疗、抗感染和改善血液循环)和局部治疗(糖尿病足清创术);在此基础上,治疗组给予脉复生合剂(由当归、熟地黄、岗捻、牛大力、两头尖、牛膝、鸡血藤、入地金牛等组成)口服治疗,对照组给予对症处理。治疗1个疗程(30 d)后观察2组临床疗效和下肢血管彩超评分疗效,比较2组治疗前后溃疡面积、腓神经运动神经传导速度(MNCV)和感觉神经传导速度(SNCV)的变化情况。【结果】治疗组的临床疗效和下肢血管彩超评分疗效均显著优于对照组(P<0.05),在缩小溃疡面积、改善MNCV和SNCV方面也显著优于对照组(P<0.05或P<0.01)。【结论】脉复生合剂治疗各期糖尿病足疗效确切,值得临床推广。

Objective To observe the therapeutic effect of Maifusheng Mixture for the treatment of diabetic foot （DF）. Methods Eighty-two DF patients were randomized into treatment group （N = 42） and control group （N = 40）. Both groups were given basic treatment including diabetic diet, insulin intensification therapy, anti- inflammatory drugs and improving blood circulation, and wound excision for diabetic foot. Additionally, the treatment group was given oral use of Maifusheng Mixture, which is mainly composed of Radix Angelicae Sinensis, Radix Rehmanniae Preparata, Radix Rhodomyrti Tomentosae, Radix Millettiae Specisoae, Rhizoma AnemonesRaddeanae, Radix Achyranthis Bidentatae, Caulis Spatholobi and Radix Zanthoxyli Nitidi, and the control group was given symptomatic treatment. After treatment for 30 days, the clinical efficiency was evaluated, and the scores of color Doppler uhrasonoscopy for lower limb blood vessels were examined in both groups. The changes of ulceration area of diabetic foot, motor nerve conduction velocity （MNCV） and sensory nerve conduction velocity （SNCV） of peroneal nerve were compared in both groups before and after treatment. Results The clinical efficiency and effect on the scores of color Doppler ultrasonoscopy for lower limb blood vessels in the treatment group were superior to those in the control group （P 〈 0.05 ） . The treatment group also had better effect on the minification of ulcer area and on the improvement of MNCV and SNCV （ P 〈 0. 05 or P 〈 0.01 ）. Conclusion Maifusheng Mixture has certain effect for the treatment of diabetic foot at different stages.
