介入治疗椎间隙感染的临床探讨

Clinical Analysis of Interventional Treatment of Intervertebral Space Infection

目的探讨介入方法治疗椎间隙感染的疗效。方法从2001年3月至2011年3月,收治椎间隙感染患者17例。其中13例患者接受介入方法治疗。通过McGill语言评价表定量动态评价患者疼痛的变化情况。定期复查X线、CT、MRI、血常规、血沉和C反应蛋白。结果接受介入手术治疗患者在术后1～3d腰痛明显缓解。3～5周症状基本消失,血沉降至20mm/L以下,C反应蛋白恢复正常。治疗后McGill分值在1～3天内开始下降。结论介入方法治疗早中期未形成较大脓肿的椎间隙感染,创伤小,费用低,恢复快,疗效确切,操作简单,是介于保守治疗与手术治疗之间的一种方法,可进可退,病人接受程度高,可达到促进患者早日康复的目的。

Objective To explore the curative effect of interventional methods in the treatment of intervertebral space infection. Methods 17 cases with intervertebral space infection were admitted from March 2001 to March 2011. 13 cases received interventional treatment. The McGill language evaluation was used to quantitatively and dynamicly evaluate the changes of the patients＇ pain. The X-ray, CT, MRI, blood count, erythrocyte sedimentation rate and C-reactive protein were regularly reviewed. Results Interventional surgical treatment of patients in the postoperative 1 to 3 d low back pain eased significantly. After 3 to 5 weeks, symptoms disappeared, erythrocyte sedimentation rate dropped to less than 20 mm/L, C-reactive protein returned to normal. McGill score in 1 to 3 days after treatment began to decline. Conclusions Interventional treatment of early and mid unforming large abscess intervertebral space infection has less trauma, low cost, quick recovery, curative effect, and simple operation, which is a method between conservative treatment and surgical treatment for patients to choose with a high degree of patient acceptance, aiming at speedy recovery of patients.
