摘要
目的探讨甲型H1N1流感患者外周血细胞计数与病情严重程度之间的关系。方法将本院300例甲型H1N1流感患者根据病情的严重程度分为普通病例组和重症及危重病例组,比较两组外周血细胞计数对甲型H1N1流感患者重症及危重病例与外周血细胞计数的相关性进行分析。结果外周血白细胞数,淋巴细胞数与预后相关(P<0.05);用Logistic回归方程对300例甲型H1N1流感患者进行回顾性验证,结合外周血白细胞数、淋巴细胞数对甲型H1N1流感患者中重症及危重病例判断的总正确率为85.1%。该方程的敏感性为70.4%,特异性为98.8%。结论外周血白细胞数、淋巴细胞数联合分析能提高对甲型H1N1流感患者中重症及危重病例判断的准确性。
Objective To investigate the relationship between peripheral blood cell counts of patients with H1N1 influenza A and severity of patients' condition. Methods 300 patients with H1N1 influenza A were divided into normal-condition group and severe-and- critical-condition group, and the peripheral blood cell counts were compared between the two groups. It used step-wise logistic regression model to analyse the relationship between the patients with severe or critical H1N1 influenza A and patients' peripheral blood cell counts. Results The counts of peripheral leukocytes and lymphocytes were correlated with patients' prognosis ( P 〈 0.05 ). All 300 patients were retrospectively analyzed using step-wise logistic regression model, which showed the overall-accuracy of estimating severe or critical condi- tion using combined analysis of peripheral peripheral leukocytes and lymphocytes counts was 85.1%. The sensibility and the specificity of the analysis equation were 70. 4% and 98. 8% respectively. Conclusion The combined analysis of peripheral leukocytes and lympho- cytes counts can improve the accuracy of estimating the severe or critical condition of patients with H1N1 influenza A.
