丙泊酚在无痛人流时对不同血压患者的影响和护理

The Influence and Nursing of Propofol treat of Patients with diffierent Basal Blood pressures in painless induced abortion

目的比较丙泊酚用于无痛人流时在不同基础血压患者中的影响,并进行观察和护理。方法该院妇产科2011年12月共进行丙泊酚麻醉下无痛人流手术186例,以基础血压100/70mmHg为基准,分低于基准值的A组和不低于基准值的B组,A组79例,B组107例;用丙泊酚2mg/kg加盐酸利卡因0.1mg/kg缓慢静脉注射,监测BP、HR、SPO2,比较两组患者的麻醉清醒时间、不良反应及监测参数的变化。结果两组患者的苏醒时间、BP、HR、SPO2的波动情况均未见显著差异,但A组患者的术后恶心、呕吐、胸闷、清醒后头晕等不良反应的发生率高于B组。结论丙泊酚麻醉下无痛人流术时,基础血压偏低的患者更容易出现不良反应;加强术中、术后的病情观察和护理工作显得至关重要。

Objective To compare Propofol＇s effect on patients with different basal blood pressures in painless induced abortion with observation and nursing.Methods Totally 186 patients through painless induced abortion under Propofol anesthesia in our Obstetrics Gynecology Department in Dec.2011 were divided into Group A and Group B on the basis of basal value 100/70mmHg：Group A 79 cases,lower than basal value 100/70mmHg;Group B 107 cases,not lower than the basal value.Both groups were intravenously injected with Propofol 2 mg/kg and lidocaine hydrochloride 0.1 mg/kg slowly to monitor BP,HR and SPO2 so as to compare variations in their awakening times after anesthesia,adverse reaction and monitored parameters.Results Both groups showed no significant difference in fluctuations of awakening time,BP,HR and SPO2.However,Group A had a higher rate of adverse reactions such as postoperative nausea,vomit,chest distress,post-awaking dizziness than Group B.Conclusion For painless induced abortion under Propofol anesthesia,those patients with lower basal blood pressure may be easier to have adverse reactions.So,it is critically important to strengthen intranperative,postoperative disease observation and nursing.
