摘要
目的:观察银屑病方治疗寻常型银屑病血热证的临床疗效及对血液流变学的影响。方法:将103例寻常型银屑病血热证患者随机分为两组,治疗组52例给予银屑病方,每日一剂,水煎服;对照组51例给予阿维A胶囊,每次10 mg,每日三次,口服。两组均12周为一疗程,一疗程后观察疗效、皮损面积和严重度指数(PASI)、瘙痒程度及不良反应。同时观察治疗前后血液流变学指标的变化。结果:治疗组总有效率98.1%,对照组总有效率100%,两组疗效比较差异无统计学意义(P>0.05);两组患者PASI评分治疗前后比较及不良反应发生率比较,差异均有统计学意义(P<0.05)。治疗组全血高切粘度(200/s)、全血中切粘度(30/s)、全血低切粘度(10/s)治疗前后比较,差异均有统计学意义(P<0.05);对照组全血高切粘度(200/s)、全血中切粘度(30/s)治疗前后比较差异均无统计学意义(P>0.05),全血低切粘度(10/s)治疗前后比较差异均有统计学意义(P<0.05)。结论:银屑病方是治疗寻常型银屑病的有效方剂;降低血液粘稠度可能是银屑病方治疗寻常型银屑病血热证的作用机制之一。
Objective:To observe the therapeutic effects of Yinxiebingfang decoction on pa- tients with psoriasis vulgaris blood-heat syndrome and the effects on hemorheology. Methods: 103 patients with psoriasis vulgaris blood-heat syndrome were randomly divided into two groups. 52 pa tients in treatment group were given one dose of Yinxiebingfang decoction every day, and the other 51 patients in control group were given 10rag of Acitretin capsules three times a day. After twelve weeks of treatment, the therapeutic effects, PASI scores, adverse reactions, and hemorheology were evaluated for the two groups. Results:The total effectiveness rate in treatment group (98.1%) was similar to that in control group (100%, P 〉0. 05). PASI scores before and after treatment showed significant differences in both groups (P 〈 0. 05 ). There was significant differ ence in incidence of adverse reactions between the two groups (P 〈 0. 05 ). Low-, middle-, and high-shear viscosity of whole blood showed significant differences before and after treatment in the treatment group (P 〈 0.05 ), while only low-shear viscosity of whole blood showed significant difference before and after treatment in the control group (P 〈 0. 05 ). Conclusion: Yinxiebingfang decoction is effective in the treatment of psoriasis vulgaris blood-heat syndrome and one of the pos- sible mechanism lies in reduction of the blood viscosity.
