老年心力衰竭与正常甲状腺病态综合征的相关性

Euthyroid sick syndrome in elderly patients with heart failure

目的观察老年心力衰竭患者ESS的发生规律及变化,以探讨甲状腺素在老年心力衰竭患者病情转归和预后中的临床意义。方法对82例不同心功能分级老年心力衰竭患者(老年心力衰竭组)及65例老年非心力衰竭患者(对照组)进行血清甲状腺素水平进行比较,观察心力衰竭缓解后的甲状腺素水平。结果与对照组相比,老年心力衰竭组血清TT3浓度明显降低,且心力衰竭程度越重,TT3水平越低。治疗后,老年心力衰竭组血清TT3水平显著升高,rT3水平降低。结论老年心力衰竭患者血清甲状腺素水平与病情严重程度和预后有关。

Objective To observe the incidence of euthyroid sick syndrome （ESS） in elderly patients with heart failure. Methods Serum thyroid hormone levels were measured in 82 elderly patients with heart failure and 65 non-heart failure elderly subjects （controls）. Results Serum TT3 concentrations in elderly heart failure patients was significantly lower than those in controls. The TT3 levels were negatively correlated with the severity of heart failure. After successful treatment the serum TT3 levels increased and TT3 levels decreased. Conclusion Serum thyroxine levels in elderly patients with heart failure are correlated with the disease severity and prognosis.
