摘要
目的:探讨幽门螺杆菌(Helicobacter pylori,H.pylori)相关性萎缩性胃炎和胃腺癌的NF-κB活性变化的相关性。方法:浅表性胃炎(H.pylori阴性)30例、H.pylori相关性萎缩性胃炎40例和胃腺癌32例;14C呼气实验检测H.pylori感染及其程度,血清抗H.pylori抗体检测既往H.pylori感染情况;凝胶迁移率变动分析(EMSA)测NF-B活性,病理检查确诊。结果:萎缩性胃炎和胃腺癌与H.pylori感染程度无关联(C系数约为0.067)。H.pylori相关性萎缩性胃炎组NF-κB活性(9343.5±1994.3)与浅表性胃炎组(7139.7±954.35)比较,差异有显著性(P<0.01);H.pylori相关性萎缩性胃炎组NF-κB活性与胃腺癌组(10919.0±3338.2)比较,差异有显著性(P<0.05)。关联性分析,C系数约为0.47。结论:H.pylori相关性萎缩性胃炎和胃腺癌的NF-B活性变化呈中度正相关,NF-B途径可能在H.pylori相关性萎缩性胃炎向胃腺癌发展过程中起一定作用。
Objective This study was to investigate the correlation of the NF-kB activity between the Helicobacter pylori (H. pylori) associated atrophic gastritis and gastric adenocarcinoma. Method 30 patients with H. pylori negative superficial gastritis, 40 patients with H. pylori associated atrophic gastritis and 32 patients with gastric adenocarcinoma were included. Serum anti-H, pylori antibody was used to detect previous H. pylori infection. Gel mobility shift assay (EMSA) was used to measure nuclear NF-kB activity. The pathology was used to confirm the diagnosis. Results There were no associations among the degree of H. pylori infection, atrophic gastritis and gastric adenocarcinoma (C coefficient is 0:067). The NF-kB activity of the H. pylori associated atrophic gastritis was significantly high than that of superficial gastritis group (9 343.5 ± 1 994.3 vs. 7 139.7 ± 954.35 ,P 〈 0.01 ) ; however the NF-kB activity of the H. pylori associated atrophic gastritis was significantly low than that of gastric adenocarcinoma( 9343.5 ± 1 994.3 vs. 9 343.5 ± 1 994.3,P 〈 0.05). Correlation analysis showed the C coefficient is 0.47. Conclusion The NF-kB activity of the H. pylori associated atrophic gastritis and gastric adenoearcinoma showed a moderate positive correlation. NF-kB pathway may play a role in the process of progression from H. pylori- related atrophic gastritis gastric adenocareinoma.
