盐酸米诺环素辅助牙周翻瓣术治疗重度慢性牙周炎临床疗效观察

Observation of Curative Effects on Minocycline Hydrochloride Assisted Periodontal Turned Flap Surgery in the Treatment of Severe Chronic Periodontitis

目的:研究盐酸米诺环素辅助牙周翻瓣术治疗重度慢性牙周炎的临床疗效。方法:共纳入13例重度慢性牙周炎患者,以牙周基础治疗后作为基线,采用分口设计方案,按照随机原则,一侧(实验侧)行翻瓣术后局部使用盐酸米诺环素一次,6周后复查并每周使用盐酸米诺环素一次,连续上药四次;另一侧(对照侧)则仅进行翻瓣术。基线和治疗后(从翻瓣术后开始计)6周、6个月测量牙龈出血指数(bleeding index,BI)、探诊深度(probing depth,PD)和附着丧失(attachment loss,AL)。结果:实验组和对照组患牙在牙周基础治疗后,即手术治疗前,各项牙周临床指标无显著性差异;两组治疗后6周和6个月时BI、PD和AL与基线比较均有明显下降;治疗后6周,两组之间临床指标BI、PD的变化有显著性差异;治疗后6个月,两组之间临床指标PD的变化有显著性差异。结论:盐酸米诺环素辅助翻瓣术在一定程度上提高了治疗效果。

Objective： To research the curative effects of minocycline hydrochloride assisted periodontal turned flap surgery in the treatment of severe chronic periodontitis. Methods： 13 patients with chronic severe periodontitis were recruited. The experimental group was subjected flap surgery combined with adjunctive application of locally delivered minocycline ointment. The control group was treated with flap surgery only. Additional minocycline application was performed at 6 weeks post operation. Periodontal indexes, such as bleeding index （BI）, probing depth （PD） and attachment loss （AL） were recorded at baseline and at 6 weeks and 6 months after treatment. Re- suits： The experimental group and the control group of teeth after periodontal therapy, the various periodontal clinical indicators was no significant difference. Both groups showed significantly clinical improvements of all clinical parameters at 6 weeks and 6 months after treat- ment compared with those at baseline. 6 weeks after treatment, clinical indicators BI, PD changes were significant differences between the two groups. Also, statistically significant difference for PD was observed between the two groups at 6 months after treatment. Conclusion ： Minocycline hydrochloride assisted periodontal tumed flap surgery can improve the treatment effect of severe chronic periodontitis.
