摘要
目的探讨早期骨梗死的磁共振成像(MRI)表现,以提高对该疾病的诊断水平。方法回顾性分析手术或穿刺证实的6例骨梗死患者的MRI表现和X线表现,重点观察病变部位、范围、在各MRI序列上的表现以及邻近软组织情况。结果 6例骨梗死共累及8个部位。股骨下段7个,胫骨上段1个,1例累及股骨和胫骨,1例双侧对称发生。6例患者X线平片均无明显异常发现。MRI显示病变位于骨干或干骺端髓腔内,T1WI呈等信号或稍低,边缘呈低信号;T2WI上病变呈等、高或混杂信号,边缘呈花边状高信号,或呈"双线征"、"三线征"。结论 X线平片对诊断早期骨梗死价值有限,MRI能够显示早期骨梗死的各种征象,是无创性诊断、评估骨梗死的最好检查方法。
Objective To explore MRI features of bone infarction in the early stage and the value of MRI in the diagnosis of bone infarction.Methods The MRI and X-ray appearances of 6 patients with bone infarction proved by surgery or biopsy were retrospectively analyzed.Position,scope of the disease,appearances in the sequence of MRI and adjacent soft tissue situation were observed.Results Among 6 cases,there were 8 sites of the bone infarction were found on MRI,7 sites were in the distal femur,1 site was in the proximal tibia.1 site was bilateral symmetry.The X-ray findings of all 6 cases were normal on plain film.MRI showed that the change was located in backbone or metaphysis epiphyseal region,the central part of infarction was manifested as slight low signal intensity on T1WI,and high signal intensity and homogeneously high signal intensity on T2WI."Double line sign" was seen in one site and "three line sign" was found in one site on T2-weighted images.Conclusion X-ray has a limited value for diagnosis of bone marrow,MRI can show the early change of bone marrow,and MRI is the best modality for the diagnosis of bone infarction in the early stage.
