复方丹参注射液治疗硬膜下出血的效果及其对脑脊液白细胞介素6的影响

Effect of Compound Danshen Injection in the treatment of subdural hemorrhage and its influence on interleukin 6 in cerebrospinal fluid

目的观察复方丹参注射液治疗急性硬膜下出血的临床疗效,并探讨对其脑脊液白细胞介素6(IL-6)水平的影响。方法将64例急性硬膜下出血患者分为观察组与对照组,观察组34例,对照组30例,对照组给予常规治疗,观察组在此基础上加用复方丹参注射液,并于不同时点观察两组治疗效果和脑脊液IL-6水平。结果①两组患者在治疗3 d、1周、2周后血肿均呈逐步缩小态势,但观察组血肿消退速度高于对照组,且差异有统计学意义(P<0.05)。②两组脑脊液IL-6水平于治疗3 d后达到峰值,1周呈逐渐下降趋势;观察组不同时点脑脊液IL-6水平均低于对照组,差异有统计学意义(P<0.05)。③用药2周后,观察组患者脑水肿治疗显效率(50.0%)与总体有效率(83.3%)均高于对照组水平(30.0%、60.0%)。结论常规治疗急性硬膜下出血基础上加用复方丹参注射液临床疗效更为显著,部分原因可能与复方丹参注射液能抑制炎性细胞因子的产生有关。

Objective To observe the clinical effect of Compound Danshen Injection in the treatment of acute subdural hemorrhage and discuss its influence on the level of interleukin 6（IL-6） in cerebrospinal fluid.Methods 64 acute subdural hemorrhage patients were divided into observation group（n = 34） and control group（n = 30）.In control group,patients were given conventional treatment,while in observation group,on the premise of conventional treatment,they were given extra compound Danshen Injection.Clinical effects and levels of IL-6 in cerebrospinal fluid in both groups were observed at different time.Results ①Hematomas in both groups were gradually reduced after the treatment of three days,one week,and two weeks.But reduction of hematoma in observation group was faster than that of control group,which showed a significant difference（P 0.05）.②After treatment for three days,IL-6 levels of cerebrospinal fluid in the two groups reached the peak,and they gradually declined one week later.At different time,IL-6 levels of cerebrospinal fluid in observation group were all lower than those of control group with a statistical difference（P 0.05）.③After treatment for two weeks,take cerebral edema as an index,obvious effective rate and total effective rate in observation group were 50.0% and 83.3% respectively,which were both superior to those of control group,30.0% and 60.0% in turn.Conclusion On the basis of conventional treatment of acute subdural hemorrhage,Compound Danshen Injection is added to obtain a remarkable clinical effect,partly due to inhibition of generation of inflammatory cytokines by Compound Danshen Injection.
