摘要
目的采用经颅彩色多普勒超声(TCCD)分析80例2型糖尿病(NIDDM)患者的血流动力学情况。方法本组80例患者,其中,单纯糖尿病10例(第1组),合并原发性高血压20例(第2组),合并冠心病16例(第3组),合并脑梗死12例(第4组),合并高脂血症22例(第5组)。采用日本ALOKA-SSD-4000型彩色多普勒超声诊断仪器,了解患者在安静时,其两侧大脑中动脉(MCA)、大脑后动脉(PCA)、基底动脉(BA)、大脑前动脉(ACA)、椎动脉(VA)的频谱形态,并比较患者峰值流速(VS)、平均流速(VAm)和搏动指数(PI)。结果糖尿病合并有高血压、冠心病、脑梗死以及高脂血症的患者其两侧MCA、PCA、BA、ACA、VA的峰值流速、平均流速和搏动指数均高于单纯糖尿病者。结论 2型糖尿病患者出现脑血管病变情况时所带来的严重性与其所患合并症有关,经颅多普勒超声对糖尿病脑血管病的早期诊断具有重要的参考价值,为评价病情的发展和预后提供重要的客观依据。
Objective To analysis the situation of hemodynamics on type 2 diabetes mellitus patients with transcranial color doppler ultrasound.Methods There were 80 patients in the group,included 10 cases of diabetes alone(the group 1),20 cases combined with hypertension(the group 2),16 cases combined with coronary heart disease(the group 3),12 cases combined with cerebral infarction(the group 4),and 22 cases combined with hyperlipidemia(the group 5).The Color doppler ultrasound diagnostic instrument(Japan ALOKA-SSD-4000)was adopted to understand the spectromorphology of both sides of the middle cerebral artery(MCA),posterior cerebral artery(PCA),basilar artery(BA),anterior cerebral artery(ACA)and vertebral artery(VA)when the patients in quiet hours,and the peak velocity(VS),mean velocity(VAm)and pulse index(PI)were compared.Results The diabetes patients combined with hypertension,coronary heart disease,cerebral infarction and hyperlipidemia in patients had higher index of peak velocity,mean velocity and pulsatility than the patients with pure diabetes in both sides of the MCA,PCA,BA,ACA,and VA.Conclusion The severity of type 2 diabetes mellitus patients supervene with cerebrovascular disease are about the complications,and transcranial doppler ultrasound has important reference value to the type 2 diabetes mellitus patients combined with cerebrovascular disease in the early diagnosis,which can provide important objective basis for the evaluation of disease progression and prognosis.
