经阴道彩色多普勒超声检测11～14孕周胎儿心脏结构的应用价值

Application value of transvaginal color doppler ultrasound in the fetal with 11-14 gestational weeks for heart structure

目的探讨经阴道彩色多普勒超声检测11～14孕周胎儿心脏结构的应用价值。方法对2011年1月～2012年5月在本院产检的11～14孕周的93例孕妇,进行经腹及经阴道彩色多普勒超声检查,重点观察胎儿心脏结构,采集胎儿心脏的四腔心切面、三血管切面及流出道切面,并测量、记录相关数据,对比两种方法在不同孕周各切面的显示率。结果经腹与经阴道彩色多普勒超声检查可显示11～14孕周的胎儿心脏结构。随着孕周的增加,各切面的显示率相应提高。经阴道彩色多普勒检查的显示率明显高于经腹部彩色多普勒检查,两者比较,差异有统计学意义(P<0.01)。结论经阴道彩色多普勒检查11～14孕周胎儿心脏结构的显示率明显高于经腹部检查,为早期检出胎儿心脏畸形提供了可靠方法,为临床拟定进一步检查及处理方案,避免严重心脏畸形胎儿出生提供诊断依据。

Objectie To study the application value of transvaginal color doppler ultrasound in the fetal with 11-14 gestational weeks for heart structure.Methods Ninety-three pregnant women with 11-14 gestational weeks from January 2011 to May 2012 were checked by the transabdominal and transvaginal color doppler ultrasound examination,the heart structure of fetal was key observed,the 4-chamber view,3-vessel view,outflow view of the heart of fetal were gathered,the correlative data were measured and recorded,the display rate of the cut surface between two methods in different gestational age was compared.Results The transabdominal and transvaginal color doppler ultrasound examination could show the heart structure in the fetal with 11-14 gestational weeks.With the increase of gestational age,the display rate of the cut surface increased accordingly.The display rate of transvaginal color doppler ultrasound examination was much higher than transabdominal color doppler ultrasound examination,there was a significant statistical difference between the results of the two tests（P0.01）.Conclusion The display rate of transvaginal color doppler ultrasound is much higher than transabdominal in the examinatiom of the fetal with 11-14 gestational weeks for heart structure,and provides a reliable method to detect the fetal heart abnormalities in early phase,and provides diagnostic basis for laying out the scheme for the further examination and dispose in clinic and avoiding severe heart abnormalities of fetal birth.
