荧光定量PCR分析儿童EB病毒感染与异型淋巴细胞相关性 被引量:6

摘要 目的了解儿童感染EB病毒(EBV)后外周血中异型淋巴细胞(异淋)的比值。方法荧光定量PCR测定外周静脉血的EBV拷贝数;确定EBV感染后,分3组:EBV感染组、住院对照组、健康对照组,各100例,取外周血涂片染色,进行白细胞分类计数,统计异淋比值。结果 EBV感染组异淋为(5.7±3.3)%,与住院对照组[(2.1±1.2)%]比较,差异有高度统计学意义(P<0.01);与健康对照组[(1.0±0.9)%]比较,差异有高度统计学意义(P<0.001);住院对照组与健康对照组比较,差异无统计学意义(P>0.05)。100例EBV感染组中有20例异淋高于10.0%,100例住院对照组中,有3例异淋高于10.0%,100例健康对照组无一例异淋高于10.0%,3组比较,差异有高度统计学意义(P<0.01)。在EBV感染组,有15.0%(15/100)异淋≤2.0%。结论 EB病毒在体内复制水平与异性淋巴细胞的比值成高度正相关,其中也存在个体差异。荧光定量PCR检测EB病毒具有特异性强、灵敏性和准确性高的优点。 Objective To understand the ratio of abnormal lymphocyte in Peripheral blood after Epstein-Barr virus (EBV) infection in children. Methods The copies of EBV in perifheral blood were determined by Fluorescence quantitative, and were divided into three groups after the infection of PCR, including EBV-infected group, hospitalized control group and health control group, and there were 100 cases in every group. Peripheral blood was taken to smear dyeing, white blood cells classification was counted, and the ratio of abnormal lymphocyte was stated. Results The proportion of atypical lym- phocytes in EBV-infected group was (5.7±3.3) %, there was significant difference between EBV-infected group and hospi- talized control group of (2.1±1.2)%, and health control group (1.0±0.9)% (P 〈 0.01) respectively. There was no significant difference between hospitalized control group and health control group (P 〉 0.05). There were 20 cases with proportion of atypical lymphocytes above 10.0% in EBV-infected group, 3 case in hospitalized control group, and no case in health control group, respectively. There was significant difference between three groups (P 〈 0.01). And there were 15.0% (15/100) of abnormal lymphocyte which was less than or equal to EBV-infected group 2.0%. Conclusion Epstein-Barr virus replica- tion levels in vivo have highly positive correlation with the ratio of atypical lymphocytes, in which there are individual dif- ferences. Detection of Epstein-Barr virus by fluorescence quantitative PCR has the advantages of high specificity, high sensitivity and high accuracy.
