定量组织速度成像对酗酒者左室舒张功能的研究

THE STUDY ON LEFT VENTRICULAR FUNCTION IN ALCOHOLICS BY USING QUANTITATIVE TISSUE VELOCITY IMAGING

目的应用定量组织速度成像评价无明显症状酗酒者的左室舒张功能。方法酗酒组31例及对照组30例。常规超声心动图测量二尖瓣口血流快速充盈速度(E),左房收缩期速度(A),计算E/A。应用实时三平面定量组织速度成像技术测量后间隔、左室侧壁、前壁、下壁、前间隔和后壁的二尖瓣环、基底段、中间段水平舒张早期峰值速度(Ve)、左房收缩期峰值速度(Va)、计算Ve/Va。并比较二组间各参数。结果酗酒者二尖瓣水平及大部分节段Ve,Ve/Va减低,Va增高(P<0.05);定量组织速度成像较脉冲多普勒对酗酒者早期左心室舒张功能异常的检出率更高(P<0.05)。结论无症状酗酒者早期即存在左心功能的异常,应用定量组织速度成像评价酗酒者左心舒张功能异常较传统超声更敏感,更可靠。

Objective To evaluate the changes of left ventrieular function in chronic asymptomatic alcoholics by using quantitative tissue velocity imaging. Methods Thirty healthy subjects as control and 31 chronic asymptomatic alcoholics were studied. Trans-mitral early （E） and late （A） Doppler flow velocities and E/A ratio were obtained by Doppler eehoeardiography. Offline LV regional velocity and dis- placement profiles along LV apical long- axis view , apical two- chamber view and four-chamber view were also obtained synchronously. The early diastole（Ve） and atrial eontraction（Va） from posterior septurn,lateral,anterior septum,posterior,anterior and inferior wall were measured and Ve /Va was calculated. The data were collected and analyzed statistically. Results There was significant difference in Ve and Ve/Va at the most parts of walls between two groups （P〈0.05）. The rate of diastolic dysfunction detected by real time triplane tissue synchronization imaging was higher than that by PD（χ2 = 4. 00, P〈 0.05）. Conclusions Left ventricular function abnormalities exists early in asymptomatic alcoholics. The quantitative tissue velocity imaging is feasible and reliable for the left ventricular function in asymptomatic alcoholics.
