注射器穿刺骨髓腔输液在心肺复苏急救中的应用 被引量:9

摘要 目的了解一次性20ml注射器穿刺骨髓腔输液在成人实施心肺复苏急救中的应用效果。方法根据实验设定的胫骨骨髓腔输液的纳入标准,采用队列研究的方法,对我科2009年12月至2011年12月实施心肺复苏急救的成人患者103例,分为实验组和对照组进行研究。实验组首选胫骨骨髓腔输液;对照组首选外周静脉输液。比较两组通道建立的时间、首次给药时间、补液速度,采用成组t检验。结果两组通道建立的时间、首次给药时间比较差异有统计学意义(P<0.05)。两组总体补液速度比较差异有统计学意义(P<0.05);骨髓腔输液通道在加压情况下,与外周静脉输液通道补液速度差异无统计学意义(P>0.05)。结论在成人实施心肺复苏术的院前及院内急救中,若外周静脉通道难以建立,采用一次性20ml注射器穿刺胫骨骨髓腔输液可达到快速给药、加压快速输液的目的。 Objective To investigate the application of 20 ml disposable syringe needle of marrow cavity infusion in cardiopulmonary resuscitation in adults. Methods According to the experimental setting of the tibial bone marrow cavity infusion into the standard, using a cohort study method, 103 adult patients scheduled for cardiopulmonary resuscitation in our department in Dec. 2009 to Dec. 2011 were divided into the study group and the control group. The study group applied tibial bone marrow cavity infusion, while the control group used peripheral intravenous infusion. The channel establishment time, the first administration time, and rehydration velocity were compared between the two group by group t test. Results The two groups showed statistically significant difference in the channel establishment time, the first administration time (P0.05), and also in rehydration velocity (P0.05). No statistically significant difference was found in the rehydration velocity of the bone marrow cavity infusion channel under a pressurized condition and the peripheral vein infusion channel (P0.05). Conclusion In cardiopulmonary resuscitation in adults, if peripheral venous access is difficult to set up, the application of 20 ml disposable syringe needle tibial bone marrow cavity infusion can achieve rapid administration and pressurized rapid infusion.
