子痫前期产妇胎盘胎膜中水通道蛋白8和水通道蛋白9的表达和意义

Expression and Significance of Aquaporin 8 and Aquaporin 9 in Fetal Membranes and Placenta with Preeclampsia

目的研究水通道蛋白8(aquaporin 8,AQP8)和水通道蛋白9(AQP9)在妊娠期高血压及子痫前期产妇胎盘胎膜上的表达,探讨其在子痫前期发病中的作用。方法选取2007年1月～2009年1月笔者医院产科足月产妇90例,其中正常妊娠30例,妊娠期高血压30例,子痫前期30例。采用免疫组化SP法,检测各组胎盘胎膜中AQP8和AQP9的表达,分析其与高血压程度、血乳酸脱氢酶水平和24h尿蛋白定量等的相关性。结果在子痫前期组,AQP8在羊膜上皮细胞的表达显著高于绒毛膜滋养细胞和胎盘滋养层细胞,而AQP9在胎盘滋养层细胞的表达显著高于绒毛膜滋养细胞和羊膜上皮细胞。子痫前期组,AQP8在羊膜上表达显著高于正常组,而在胎盘上的表达显著下降;AQP9在胎盘上表达显著高于正常组,在羊膜上表达轻度升高。子痫前期产妇胎盘上AQP8,AQP9的表达与血乳酸脱氢酶水平相关,而与24h尿蛋白水平无关。结论 AQP8、AQP9在子痫前期产妇胎盘胎膜上表达异常,可能参与子痫前期的发生和发展。

Objective To study the expression of aquaporin 8 and 9 in fetal membranes and placenta with preeclampsia and gesta- tional hypertension and investigate the role of aquaporin 8 and 9 in preeelampsia. Methods From Jan 2007 to Jan 2009, 90 patients who had elective cesarean deliveries within our division of obstetrics were studied, in which 30 patients with preeclampsia, 30 patients with gestational hypertension and 30 patients with normal pregnancy. The different expressions of aquaporin 8 and 9 in fetal membranes and placenta among the three groups were detected using immunohistochemical staining. The correlation between the expressions of aquaporin 8 and 9 and hypertension, 24 - hour urinary protein and lactic dehydrogenase level in blood were also investigated. Results In pre- eclampsia group, the expression of aqnaporin 8 in the amniotic membrane was significantly higher than that in the trophoblasts of placenta and chorion, while the expression of aquaporin 9 in the trophoblasts of placenta was significantly higher than that in the amniotic membrane and chorion trophoblasts. Compared with normal group, the expression of aquaporin 8 in the amniotic membrane in preeelampsia group was significantly increased, while its expression in the placenta was significantly decreased, but the expression of aquaporin 9 in the placenta was significantly increased, and its expression in amniotic membrane was slightly increased. In preeclampsia group, the expression of aquaporin 8 and 9 in the placenta was correlated with the level of lactic dehydrogenase in blood, but not correlated with 24 - hour urinary protein excretion. Conclusion The abnormal expressions of aquaporin 8 and 9 in the placenta and fetal membranes with preeclampsia in- dicated that they may participate in the occurrence and development of preeclampsia.
