重硅油填充治疗下方视网膜脱离的疗效 被引量:8

摘要 目的观察重硅油填充治疗下方视网膜脱离的临床效果。方法回顾性分析收治的24例24眼下方视网膜脱离患者,有裂孔者位于3~11点。均采用玻璃体切除和重硅油填充术,术中依病情需要联合膜剥离、视网膜切开、重水应用、眼内激光、白内障超声乳化及人工晶体植入、普通硅油取出术等。观察术后视网膜复位、视力、眼压及并发症情况。结果随访2~13个月,平均(6.3±3.28)个月。手术后2周视网膜均复位(100%),末次随访初次手术复位21眼(87.5%),二次手术复位22眼(91.7%)。术后最佳矫正视力与术前比较差异有统计学意义(t=3.07,P<0.05)。其中17眼(70.8%)最佳矫正视力提高,13眼(54.2%)在0.05以上。并发白内障100%(除3眼无后囊外);11眼(45.8%)术后高眼压;9眼(37.5%)术后并发增殖膜;重硅油乳化和角膜变性率较低,分别为3眼(12.5%)和1眼(4.2%)。结论重硅油作为眼内填充物治疗合并下方视网膜脱离者效果较好,主要并发症有白内障、高眼压和增殖膜。
