摘要
目的:研究卒中后抑郁(PSD)患者外周血单核细胞(PBMC)趋化因子受体1(CX3CR1)、锌指蛋白A20表达及甲状腺功能的变化。方法:PSD组为46例PSD患者,对照组为50例同期住院的老年缺血性卒中患者。入院后d 14分别检测两组患者的外周血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素(TSH)浓度及PBMC CX3CR1和锌指蛋白A20表达的变化。结果:对照组外周血清FT3和TSH浓度较PSD组显著升高,差异有统计学显著性(P<0.05);PSD组外周血清FT4浓度和PBMCCX3CR1表达较对照组明显升高,差异有统计学显著性(P<0.05)。PSD组汉密尔顿(HAMD)评分与FT4浓度和PBMC CX3CR1表达成显著正相关(P<0.05),与FT3及TSH浓度成显著负相关(P<0.05)。结论:外周血清FT3,FT4,TSH浓度及PBMC CX3CR1表达与PSD关系密切,可能是老年PSD患者重要的分子标志物。
Objective: To determine the expression of CX3C-chemokine receptor 1 (CX3CR1) and zinc finger protein A20 in peripheral blood mononuclear cells (PBMC) and the changes of serum thyroid hormones in old patients with post-stroke depression (PSD). Methods: A total of 46 old PSD patients were enrolled, and 50 older hospitalized patients with acute ischemic stroke but without depression were selected as the control. The changes in serum concentrations of free triiodothyronine (FT3) , free thyroxine (-FT4) and thyroid-stimulating hormone (TSH) , and the expression of CX3CR1 and zinc finger protein A20 in PBMC were detected, respectively, at 14 days after admission. Results: Serum FT3 and TSH concentrations were obviously lower in PSD patients than in control patients (P 〈 0.05); serum FT4 concentration and the expression of CX3CR1 in PBMC were obviously higher in PSD patients than in control patients (P 〈 0.05). Hamilton depression scale (HAMD) was positively correlated with serum FT4 concentration and the expression of CX3CR1 in PBMC (P 〈 0.05) , and it was negatively correlated with FT3 and TSH concentrations (P 〈 0.05) in PSD patients. Conclusion: FT3, FT4 and TSH concentrations in serum and the expression of CX3CR1 in PBMC are closely associated with PSD. They may be the important molecular markers of PSD in old stroke patients.
