CTLA-4单核苷酸多态性对肾移植受者近期急性排斥反应发生的影响

摘要 目的了解和评价细胞毒T淋巴细胞相关抗原4(cytotoxicTlymphocyteassociatedantigen-4,CTLA-4)蛋白的基因多态性位点-1722T/C、-1661A/G、-318C/T、+49A/G、+6230G/A对肾移植近期急性排斥反应的影响。方法采用测序法对上海市器官移植中心167例首次尸体肾移植受者进行CTLA-4-1722T/C、-1661A/G、-318C/T、+49A/G、+6230G/A位点检测。结果①肾移植术后6个月内,167例。肾移植受者45例发生急性排斥反应,发生率为26.9%,第1次排斥反应发生时间为术后5-140天,所有急性排斥反应经过抗排斥治疗均能逆转。②排斥组和非排斥组比较CTLA-4各个位点分布不同,有统计学意义的基因型为-1722TT。③各个位点的等位基因检测中,有统计学意义的等位基因为-1722T。结论CTLA-4中基因型-1722TT等位基因-1722T与肾移植受者近期急性排斥反应有关,可能为急性排斥反应的危险因素。 Objective To understand and evaluate the influence of eytotoxic T lymphocyte associated antigen -4 ( CTLA - 4) gene polymorphism - 1722T/C, - 1661A/G, - 318C/T, + 49A/G, + 6230 G/A on early acute rejection of kidney transplants. Methods These loci were detected by sequencing, from which 167 recipients accepted eadaverie renal transplantation for the first time in Shanghai Organ Transplantation Center. Results①6 months after transplanta- tion, among 167 cases of kidney transplant recipients, 45 cases suffered from early acute rejection of kidney transplants (26.9%). The first rejection occurred 5 - 140 days after transplantation. All acute rejections were reversed after anti - rejection therapy. ②Acute rejection group and non - acute rejection group had different distribution of various sites in CTLA- 4 and the significant genotype was -1722TT. ③ Various alleles were detected, with significant allele being - 1722T. Conclusion CTLA -4 genotype - 1722TT and allele - 1722T are related with early acute rejection of kidney transplants, and are possible risk factors for acute rejection.
