摘要
目的探讨乙酰肝素酶(Heparanase,Hpa)在慢性肾脏疾病患者肾组织中的表达分布情况及其与临床表现的关系。方法分别采用非生物素免疫组化二步法、SABC法检测36例慢性肾脏疾病患者肾小球和肾小管间质乙酰肝素酶的表达水平,并结合临床资料及肾脏病理进行统计学分析。结果乙酰肝素酶在对照组正常肾组织的肾小球基底膜、肾小球足细胞、系膜细胞及肾小管上皮细胞有少量表达。而在慢性肾脏疾病患者的病变肾组织中,Hpa表达显著增强,与正常对照组相比,慢性肾脏疾病患者肾组织中肾小球和肾小管上皮细胞均有Hpa表达,其表达水平与对照组相比明显升高,具有显著统计学差异(P〈0.01)。Hpa和临床指标的相关性:Hpa与24h尿蛋白定量呈正相关关系,r=0.541(P〈0.01)。有统计学意义;与血浆白蛋白呈负相关关系,r=-0.694(P〈0.01),有统计学意义;与血肌酐、血脂、免疫球蛋白无相关性。结论Hpa的表达与蛋白尿的程度存在正相关,提示Hpa作为一种与蛋白尿密切相关的酶,已越来越受到人们的重视,他将成为蛋白尿治疗的新靶点。
Objectives To explore and disscuss the express division of Heparanase in the chronic nephropathy patients and its connection with clinical situation. Methods Checking the expressive level of Heparanase of acinus renis and nephric tubule of 36 chronic nephropathy patients through two stages of non - bioepiderm immunity class and SABC , then, making statistics analysis with the clinical information and patho of nephropathy. Results Heparanase takes on a little express in basal lamina and cells of aeinus renis ,intercapillary cells and cellula epithe- lialis nephric tubule of the nephros tissue in Contrastive Control Group. Contrasted with the normal Control Group , Hpa obviously increases in the affection nephridial tissue of the chronic nephropathy patient and appears in the acinus renis and nephric tubule , and contrasted with the Control Group, it has the statistics difference ( P 〈 0.01 ). De- pendablity between Hpa and clinical index : Hpa has positive correlation with 24 hour Urine protein quantitation r = 0. 5407 ( P 〈 0.01 ), negative correlation with plasma - albumin r = - 0. 6948 ( P 〈 0.01 ) , which are statistics sig- nificance. It has no dependablity with serum creatinine, blood fat, immune globulin. Conclusions The express of Hpa has positive correlation with albuminuric extent, which demostrates that Hpa , one enzyme closely connected with albuminuria, has been arising the interests and reconstruction of the public and will become a new target of albu- minuric therapy.
